# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic and antimuscarinic properties.
## Primary Indications
* Gastrointestinal (GI) antispasmodic
## Adult Dosing
* **GI Antispasmodic:** 30 mg orally every 4 hours as needed.
* **Maximum Dose:** Not explicitly established for GI indications.
## Pediatric Dosing
* Information on pediatric dosing is not readily available or established. Use with caution and at the discretion of the prescriber.
## Dose Adjustments
* **Renal Impairment:** No specific dosing adjustments are established. Use with caution.
* **Hepatic Impairment:** No specific dosing adjustments are established. Use with caution.
## Contraindications
* Hypersensitivity to walethmade bromide or other quaternary ammonium compounds.
* Conditions where anticholinergic effects are undesirable, such as:
* Glaucoma
* Obstructive uropathy (e.g., bladder neck obstruction, prostatic hypertrophy)
* Obstructive GI disease (e.g., pyloric stenosis, achalasia)
* Myasthenia gravis
* Uncontrolled tachycardia
## Adverse Effects
* **Common:** Dry mouth, blurred vision, constipation, urinary retention, decreased sweating (anhidrosis), drowsiness, dizziness, tachycardia.
* **Less Common/Serious:** Confusion, hallucinations (especially in the elderly), paralytic ileus, severe tachycardia, increased intraocular pressure.
## Key Drug Interactions
* **Other Anticholinergics:** Additive anticholinergic effects.
* **Amantadine:** Additive anticholinergic effects.
* **Antacids:** May decrease absorption of walethmade bromide. Administer walethmade bromide at least 1 hour before or 2 hours after antacids.
* **Opioid Analgesics:** Increased risk of paralytic ileus.
* **Potassium Chloride:** Oral potassium chloride may cause gastrointestinal lesions; risk is increased with anticholinergics.
## Monitoring
* Monitor for signs and symptoms of anticholinergic effects (e.g., dry mouth, blurred vision, urinary retention, constipation, drowsiness).
* Monitor for signs of hypersensitivity.
* In elderly patients, monitor for confusion or delirium.
## Clinical Pearls
* Due to its quaternary ammonium structure, walethmade bromide has limited penetration into the central nervous system, potentially leading to fewer CNS side effects compared to other anticholinergics, although CNS effects can still occur.
* Advise patients to avoid activities requiring mental alertness or coordination until they know how the drug affects them.
* Advise patients to report any signs of urinary retention, severe constipation, or vision changes.
