# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic properties.
## Primary Indications
Walethmade bromide is indicated for the management of peptic ulcers and irritable bowel syndrome.
## Adult Dosing
* **Peptic Ulcers:** 10-20 mg orally three to four times daily, 30 minutes before meals and at bedtime.
* **Irritable Bowel Syndrome:** 10-20 mg orally three to four times daily.
Maximum daily dose: 80 mg.
## Pediatric Dosing
There is no established pediatric dosing for Walethmade bromide.
## Dose Adjustments
* **Hepatic Impairment:** No specific dose adjustments are routinely recommended, but caution is advised due to potential accumulation.
* **Renal Impairment:** No specific dose adjustments are routinely recommended, but caution is advised due to potential accumulation.
## Contraindications
* Glaucoma
* Prostatic hypertrophy
* Obstructive uropathy
* Obstructive gastrointestinal tract disease
* Myasthenia gravis
* Known hypersensitivity to walethmade bromide or other anticholinergic agents.
## Adverse Effects
Common adverse effects include dry mouth, blurred vision, constipation, urinary retention, drowsiness, confusion, and tachycardia. Less common effects include dizziness, nausea, vomiting, and skin rash.
## Key Drug Interactions
* **Other Anticholinergic Agents:** Increased risk of additive anticholinergic side effects.
* **Antacids:** May decrease the absorption of walethmade bromide. Administer antacids at least 1 hour apart from walethmade bromide.
* **Opioids:** Increased risk of severe constipation and ileus.
* **MAO Inhibitors:** May potentiate anticholinergic effects.
* **Cholinergic Agents:** Antagonistic effects.
## Monitoring
* Monitor for signs and symptoms of anticholinergic adverse effects (e.g., dry mouth, blurred vision, urinary retention, constipation).
* Assess for efficacy in symptom control.
## Clinical Pearls
* Administer 30 minutes before meals to reduce gastric acid secretion.
* Due to its anticholinergic effects, caution is advised in elderly patients who are more susceptible to CNS side effects and urinary retention.
* Patients should be advised to avoid activities requiring mental alertness until they know how this medication affects them.
