# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic and antispasmodic properties.
## Primary Indications
* Adjunctive treatment of peptic ulcers.
* Gastrointestinal spasms.
## Adult Dosing
* **Peptic Ulcer Disease:** 20 mg orally four times daily, 30 minutes before meals and at bedtime.
* **Gastrointestinal Spasms:** 20 mg orally four times daily.
The maximum daily dose is generally not specified but titrate to effect and tolerability.
## Pediatric Dosing
Dosing has not been established in pediatric populations.
## Dose Adjustments
No specific dose adjustments are typically recommended for renal or hepatic impairment, but caution is advised due to potential accumulation.
## Contraindications
* Hypersensitivity to walethmade bromide or other anticholinergics.
* Glaucoma.
* Prostatic hypertrophy.
* Obstructive uropathy.
* Obstructive gastrointestinal tract disease (e.g., achalasia, pyloric stenosis).
* Myasthenia gravis.
* Unstable cardiovascular status in acute hemorrhage.
## Adverse Effects
Common adverse effects are anticholinergic in nature and include dry mouth, blurred vision, dilated pupils, urinary retention, constipation, dizziness, drowsiness, and tachycardia.
## Key Drug Interactions
* **Amantadine, Tricyclic Antidepressants, MAO Inhibitors, Some Antihistamines, Phenothiazines, Opioid Analgesics:** Increased anticholinergic effects.
* **Corticosteroids:** May increase intraocular pressure.
* **Nitrates:** Reduced efficacy of nitrates due to decreased salivation.
* **Potassium Chloride:** Increased risk of gastrointestinal ulceration when taken concomitantly.
## Monitoring
* Monitor for signs and symptoms of anticholinergic side effects.
* Assess for improvement in ulcer symptoms or GI spasms.
* Evaluate for potential drug interactions.
## Clinical Pearls
* Administer 30 minutes before meals to maximize efficacy for peptic ulcer disease.
