# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic properties.
## Primary Indications
Historically used for gastrointestinal disorders such as peptic ulcers and spasms. Current use is limited due to newer, more effective agents.
## Adult Dosing
Dosing is not well-established for current use. Historically, doses varied, but common starting points were around 10-20 mg orally three times daily. Maximum doses were generally not clearly defined in older literature.
## Pediatric Dosing
No established pediatric dosing.
## Dose Adjustments
Dose adjustment for renal or hepatic impairment is not well-defined. Caution is advised in elderly patients due to increased sensitivity to anticholinergic effects.
## Contraindications
- Known hypersensitivity to Walethmade bromide.
- Conditions where anticholinergic effects are detrimental, such as:
- Glaucoma (especially narrow-angle)
- Myasthenia gravis
- Severe ulcerative colitis or toxic megacolon
- Untreated urinary retention or bladder neck obstruction
- Severe cardiac disease or arrhythmias
## Adverse Effects
Common anticholinergic side effects:
- Dry mouth
- Blurred vision
- Constipation
- Urinary retention
- Drowsiness
- Tachycardia
Less common:
- Confusion
- Agitation
- Nausea, vomiting
## Key Drug Interactions
- **Other anticholinergic agents:** Additive anticholinergic effects.
- **Monoamine oxidase inhibitors (MAOIs):** May potentiate anticholinergic effects.
- **Potassium chloride:** Increased risk of gastrointestinal ulceration and bleeding.
- **Amantadine:** Additive anticholinergic effects.
- **Dopaminergic agents:** May reduce the efficacy of agents like levodopa.
- **Antacids and antidiarrheals:** May decrease absorption; administer Walethmade bromide at least 1 hour before or after these agents.
## Monitoring
- Monitor for efficacy in symptom relief.
- Monitor for signs and symptoms of anticholinergic toxicity (e.g., dry mouth, blurred vision, urinary retention, constipation, confusion).
## Clinical Pearls
- Due to limited current use and availability of safer alternatives, Walethmade bromide is rarely prescribed.
- Anticholinergic side effects can be particularly problematic in the elderly.
- Administration with food may reduce gastrointestinal upset, but may also impact absorption; timing relative to other medications is important.
