# Walethmade bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic properties. It is poorly absorbed from the gastrointestinal tract.
## Primary Indications
* Gastrointestinal spasms
* Biliary spasms
* Ureteral spasms
## Adult Dosing
* **Gastrointestinal/Biliary Spasms:** 50 mg to 100 mg orally every 4 to 6 hours as needed.
* **Ureteral Spasms:** 50 mg to 100 mg orally every 4 to 6 hours as needed.
## Pediatric Dosing
There is no established pediatric dosing for Walethmade bromide. Use in pediatric populations is generally not recommended.
## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment due to poor absorption. However, caution is advised in elderly patients due to potential for increased sensitivity to anticholinergic effects.
## Contraindications
* Myasthenia gravis
* Glaucoma (narrow-angle)
* Prostatic hypertrophy
* Paralytic ileus
* Tachycardia
* Gastrointestinal obstruction
* Known hypersensitivity to Walethmade bromide or bromides.
## Adverse Effects
Common anticholinergic effects: dry mouth, blurred vision, constipation, urinary retention, drowsiness, confusion.
## Key Drug Interactions
* **Other Anticholinergics:** Additive anticholinergic effects.
* **CNS Depressants:** May enhance sedative effects.
* **Potassium Supplements:** Use with caution, may increase risk of gastrointestinal ulceration.
## Monitoring
* Monitor for signs and symptoms of anticholinergic side effects.
* Assess for relief of symptoms.
## Clinical Pearls
* Due to poor absorption, systemic effects are generally limited.
* The bromide ion can accumulate with chronic use, potentially leading to bromism (neurologic symptoms, skin rash).
