# Walethmade Bromide
## Overview
Walethmade bromide is an antiepileptic medication used for the management of certain types of seizures.
## Primary Indications
* Adjunctive therapy for partial-onset seizures in patients 12 years of age and older.
## Adult Dosing
* **Starting Dose:** 100 mg twice daily.
* **Titration:** Increase by 100 mg twice daily every week.
* **Maintenance Dose:** Typically 200 mg to 600 mg twice daily.
* **Maximum Dose:** 1200 mg twice daily (2400 mg/day).
## Pediatric Dosing
* **Patients 12 to 17 years of age:** Same as adult dosing. Dosing is based on weight, with a maximum dose of 1200 mg twice daily.
* **Data for pediatric patients younger than 12 years is limited.**
## Dose Adjustments
* **Renal Impairment:** Dose reduction is necessary based on creatinine clearance (CrCl).
* CrCl 30-60 mL/min: Reduce dose by 50%.
* CrCl < 30 mL/min: Reduce dose by 75%.
* **Hepatic Impairment:** No specific dose adjustment guidelines are established, but caution is advised.
## Contraindications
* Known hypersensitivity to walethmade bromide or its components.
## Adverse Effects
* **Common:** Dizziness, somnolence, headache, nausea, fatigue, diplopia, ataxia.
* **Serious:** Suicidal ideation and behavior, psychiatric effects (hallucinations, psychosis), hypersensitivity reactions.
## Key Drug Interactions
* **CYP Enzyme Inducers (e.g., carbamazepine, phenytoin, rifampin):** May decrease walethmade bromide levels. Monitor for reduced efficacy.
* **CYP Enzyme Inhibitors (e.g., ketoconazole):** May increase walethmade bromide levels. Monitor for increased adverse effects.
* **CNS Depressants (e.g., alcohol, benzodiazepines):** Additive CNS depressant effects.
## Monitoring
* Monitor for signs of suicidal ideation and behavior.
* Monitor for psychiatric symptoms.
* Assess for therapeutic response and adverse effects.
* Renal function should be assessed and dose adjusted accordingly.
## Clinical Pearls
* Titrate dose slowly to minimize adverse effects, particularly CNS depression.
* Walethmade bromide may cause gastrointestinal upset; consider taking with food.
* Patients should be advised against driving or operating heavy machinery until they know how walethmade bromide affects them.
