# Walethmade Bromide
## Overview
Walethmade bromide is an anticholinergic medication.
## Primary Indications
* Treatment of Parkinson's disease symptoms (tremor, rigidity).
* Management of drug-induced extrapyramidal symptoms.
## Adult Dosing
* **Parkinson's disease:** Initiate at 1 mg orally once or twice daily, increasing gradually as tolerated. Usual maintenance dose is 1-2 mg three times daily. Maximum dose is generally 6 mg daily, though some sources suggest up to 10 mg daily in select cases.
* **Drug-induced extrapyramidal symptoms:** 1-2 mg orally once or twice daily, as needed.
## Pediatric Dosing
Dosing in pediatric patients is not well-established. Use with caution and at the lowest effective dose if necessary.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution; no specific dose adjustment guidelines are widely established, but reduced doses may be necessary.

## Contraindications

## Adverse Effects
Common anticholinergic side effects: dry mouth, blurred vision, constipation, urinary retention, confusion, drowsiness, dizziness, nausea, vomiting, tachycardia. Less common: hallucinations, memory impairment, fever, heat prostration.
## Key Drug Interactions
* **Other anticholinergics:** Additive anticholinergic effects.
* **CNS depressants (e.g., alcohol, sedatives, opioids):** Increased CNS depression.
* **MAO inhibitors:** May potentiate anticholinergic effects.
* **Amantadine:** Increased risk of anticholinergic and CNS adverse effects.
* **Potassium chloride:** Increased risk of gastrointestinal ulceration when taken with solid oral dosage forms.
## Monitoring
* Monitor for improvement in Parkinsonian symptoms or extrapyramidal symptoms.
* Assess for adverse effects, particularly anticholinergic side effects, cognitive changes, and urinary retention.
* Monitor for signs of glaucoma exacerbation in susceptible individuals.
## Clinical Pearls
* Gradual titration is essential to minimize side effects.
* May be more effective for tremor and rigidity than for bradykinesia.
* Can be used adjunctively with levodopa to manage Parkinson's disease.
* Educate patients on potential side effects, especially dry mouth (recommend sugar-free candy or gum) and constipation (recommend increased fiber and fluids).
