# Walethmade Bromide
## Overview
Walethmade bromide is a non-barbiturate hypnotic and sedative agent. It is a bromide salt with central nervous system depressant properties.
## Primary Indications
* Insomnia
## Adult Dosing
* **Insomnia:** 50 mg to 100 mg orally at bedtime.
* Maximum dose: 100 mg per dose.
## Pediatric Dosing
* Dosing in pediatric patients is not established. Use with caution if considered.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. Consider reduced dose.
* **Renal Impairment:** Use with caution. Monitor for accumulation.
* **Elderly:** Use with caution. Lower doses may be appropriate due to increased sensitivity.
## Contraindications
* Hypersensitivity to walethmade bromide or other bromide salts.
* Severe hepatic or renal impairment.
* Severe respiratory depression.
* Acute intermittent porphyria.
## Adverse Effects
* **Common:** Drowsiness, dizziness, headache, ataxia, confusion, gastrointestinal upset.
* **Serious:** Bromism (chronic toxicity from bromide accumulation), characterized by neurological (lethargy, stupor, psychosis, memory impairment), psychiatric, and dermatological symptoms (acneiform rash). Respiratory depression, paradoxical excitement, hypotension.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, benzodiazepines, other sedatives/hypnotics):** Additive CNS depression. Increased risk of sedation, respiratory depression.
* **Drugs affecting hepatic metabolism (CYP450 inducers/inhibitors):** Potential for altered walethmade bromide levels, although its metabolism is not extensively characterized.
## Monitoring
* Signs and symptoms of bromism, especially with prolonged use or in patients with impaired renal function.
* Sedation and cognitive function.
* Respiratory rate and depth.
## Clinical Pearls
* Due to the risk of bromism, use should be limited to short-term treatment.
* Tolerance may develop, necessitating dose escalation, which increases the risk of toxicity.
* Avoid concurrent use with other CNS depressants.
* Consider alternative agents for chronic insomnia management due to the potential for dependence and toxicity.
