# Walethmade bromide
## Overview
Walethmade bromide is an anticholinergic medication.
## Primary Indications
* Management of motion sickness.
* Management of nausea and vomiting.
## Adult Dosing
* **Motion Sickness:** 10 mg to 25 mg orally every 4 hours as needed, not to exceed 100 mg in 24 hours.
* **Nausea and Vomiting:** 10 mg to 25 mg orally every 4 hours as needed.
## Pediatric Dosing
Dosing in pediatric patients is not well established and should be determined by a healthcare provider based on clinical assessment.
## Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment. However, caution and potential dose reduction may be considered in patients with severe impairment due to potential accumulation.
## Contraindications
* Hypersensitivity to scopolamine or other anticholinergic agents.
* Angle-closure glaucoma.
* Conditions that might be aggravated by anticholinergic effects (e.g., pyloric obstruction, cardiospasm, myasthenia gravis, severe ulcerative colitis, toxic megacolon).
## Adverse Effects
Common adverse effects include dry mouth, drowsiness, blurred vision, dizziness, and confusion. Less common effects include urinary retention, constipation, tachycardia, and disorientation.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, sedatives, hypnotics, opioids):** May potentiate CNS depressant effects, leading to increased drowsiness and impaired coordination.
* **Other Anticholinergic Agents:** Increased risk of anticholinergic side effects (e.g., dry mouth, blurred vision, constipation, urinary retention).
* **MAO Inhibitors:** May prolong and intensify anticholinergic effects.
## Monitoring
* Monitor for signs and symptoms of anticholinergic toxicity, particularly in the elderly or those with renal impairment.
* Assess for effectiveness in controlling nausea, vomiting, or motion sickness.
* Evaluate for adverse effects such as drowsiness, confusion, blurred vision, and urinary retention.

* Transdermal scopolamine patches are available for prolonged motion sickness prophylaxis and may offer a more consistent delivery profile.
* Due to potential for drowsiness, patients should be advised to avoid operating heavy machinery or driving until they know how this medication affects them.
* Use with caution in the elderly, as they are more susceptible to anticholinergic side effects.
