# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic properties. It is thought to reduce gastric motility and secretions.
## Primary Indications
* Gastric motility disorders.
* Peptic ulcer disease (adjunctive therapy).
* Irritable bowel syndrome.
## Adult Dosing
* **Gastric motility disorders/Peptic ulcer disease:** 5-10 mg orally three times daily.
* **Irritable bowel syndrome:** 5 mg orally two to three times daily.
* Maximum dose: Not clearly established, but doses exceeding 30 mg daily are generally not recommended.
## Pediatric Dosing
Limited data are available for pediatric use. Dosing should be individualized under specialist guidance.
## Dose Adjustments
* **Hepatic Impairment:** No specific dose adjustment guidelines are available. Use with caution.
* **Renal Impairment:** No specific dose adjustment guidelines are available. Use with caution.
## Contraindications
* Myasthenia gravis.
* Glaucoma.
* Obstructive uropathy.
* Obstructive gastrointestinal disease.
* Known hypersensitivity to walethmade bromide.
## Adverse Effects
Common adverse effects include dry mouth, blurred vision, constipation, urinary retention, and drowsiness. Less common effects include increased heart rate, confusion, and dizziness.
## Key Drug Interactions
* **Other Anticholinergics:** Additive anticholinergic effects, increasing risk of adverse events.
* **Central Nervous System (CNS) Depressants:** May potentiate CNS depressant effects.
* **Proton Pump Inhibitors (PPIs):** Some evidence suggests potential for reduced efficacy of PPIs with concomitant use, though clinical significance is uncertain.
## Monitoring
* Monitor for signs and symptoms of anticholinergic adverse effects (e.g., dry mouth, urinary retention, constipation, blurred vision, confusion).
* Assess for therapeutic response.
## Clinical Pearls
* Walethmade bromide may impair the ability to perform tasks requiring mental alertness, such as driving or operating machinery.
* Ensure adequate hydration and fiber intake to mitigate constipation.
* This agent is often used as an adjunct and not typically as monotherapy for serious conditions.
