# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic and antispasmodic properties. It is primarily used to reduce smooth muscle spasms.
## Primary Indications
* Symptomatic relief of gastrointestinal spasms and hypermotility.
* Adjunct in the treatment of peptic ulcer disease.
## Adult Dosing
* **Oral:** 15-30 mg three times daily, taken 30 minutes before meals and at bedtime. Maximum daily dose is not well-established but generally kept below 120 mg/day.
## Pediatric Dosing

* **Potassium Chloride:** Increased risk of gastrointestinal ulceration and bleeding when co-administered with potassium chloride preparations.
* **Proton Pump Inhibitors (PPIs) / H2 Blockers:** May decrease the efficacy of PPIs and H2 blockers by increasing gastric pH and reducing the need for acid suppression.
## Monitoring
* Monitor for signs of anticholinergic side effects (dry mouth, blurred vision, urinary retention, constipation).
* Monitor for signs of CNS effects (drowsiness, confusion).
* Assess efficacy in relieving gastrointestinal symptoms.
## Clinical Pearls
* Administer 30 minutes before meals and at bedtime to maximize efficacy for GI spasms.
* Advise patients to avoid activities requiring mental alertness until they know how this medication affects them.
* Caution patients regarding potential for heat prostration in hot weather due to decreased sweating.
