# TUSQD
## Overview
TUSQD is a combination medication containing hydrocodone bitartrate and guaifenesin. Hydrocodone is an opioid agonist that acts as a cough suppressant. Guaifenesin is an expectorant that helps to loosen mucus in the airways.
## Primary Indications
* Symptomatic relief of cough and congestion associated with the common cold, bronchitis, or other respiratory conditions.
## Adult Dosing
* **Recommended dose:** 10 mL (containing hydrocodone bitartrate 5 mg and guaifenesin 100 mg per 5 mL) orally every 4 to 6 hours as needed.
* **Maximum dose:** Not to exceed 40 mL in 24 hours.
## Pediatric Dosing
* **Children 6 to 12 years:** 5 mL (containing hydrocodone bitartrate 2.5 mg and guaifenesin 50 mg per 5 mL) orally every 4 to 6 hours as needed.
* **Children under 6 years:** Safety and efficacy have not been established.
## Dose Adjustments
* **Renal or Hepatic Impairment:** Use with caution. Dosing adjustments may be necessary based on individual patient response and clinical judgment. Specific guidelines are not established.
## Contraindications
* Hypersensitivity to hydrocodone, guaifenesin, or any component of the formulation.
* Respiratory depression.
* Severe asthma or other conditions where bronchospasm may be induced.
* Concurrent use with MAO inhibitors or within 14 days of stopping MAO inhibitors.
## Adverse Effects
* **Common:** Drowsiness, dizziness, constipation, nausea, vomiting, dry mouth, pruritus.
* **Serious:** Respiratory depression, abuse, dependence, addiction, opioid-induced hyperalgesia, significant central nervous system (CNS) depression.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, benzodiazepines, sedatives):** Increased risk of profound sedation, respiratory depression, coma, and death.
* **MAO Inhibitors:** Potentially severe and unpredictable reactions.
* **CYP3A4 Inhibitors (e.g., macrolide antibiotics, azole antifungals):** Increased hydrocodone levels, leading to increased opioid effects and adverse reactions.
## Monitoring
* Respiratory rate and depth.
* Level of consciousness.
* Bowel function (for constipation).
* Signs of opioid abuse or dependence.
## Clinical Pearls
* Due to the opioid component, TUSQD has a potential for abuse, dependence, and addiction. Prescribe with caution and consider the individual patient's risk factors.
* Advise patients to avoid activities requiring mental alertness (e.g., driving, operating machinery) until they know how the medication affects them.
* Promote adequate hydration to aid guaifenesin's expectorant effect.
* This medication should be used for the shortest duration necessary.
