## TUSQ-QD
### Overview
TUSQ-QD is a brand name for a combination product containing **guaifenesin** and **dextromethorphan**. Guaifenesin is an expectorant that helps thin and loosen mucus. Dextromethorphan is a cough suppressant that acts on the cough center in the brain.
### Primary Indications
* Temporary relief of cough due to minor throat and bronchial irritation.
* Temporary relief of nasal congestion.
* Temporary relief of sinus congestion and pressure.
* Temporary relief of cough associated with colds, bronchitis, and other respiratory conditions.
### Adult Dosing
* **Immediate-release formulations:** 10 mL to 20 mL orally every 4 hours as needed.
* **Maximum dose:** Consult product labeling; generally, not to exceed 120 mL in 24 hours. Specific dosing may vary by product strength and formulation.
### Pediatric Dosing
* **Children 6 to under 12 years:** 5 mL to 10 mL orally every 4 hours as needed.
* **Children 2 to under 6 years:** 2.5 mL to 5 mL orally every 4 hours as needed.
* **Children under 2 years:** Use is generally **not recommended** without physician guidance due to increased risk of adverse effects. Consult product labeling for specific age recommendations and dosing.
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. Consider reduced dosage and increased dosing interval. Data on specific adjustments is limited.
* **Renal Impairment:** Use with caution. Data on specific adjustments is limited.
### Contraindications
* Hypersensitivity to guaifenesin or dextromethorphan.
* Use within 14 days of MAOI therapy.
* Asthma, emphysema, chronic bronchitis, or persistent cough (unless directed by a physician).
* Patient taking prescription nitrosamine inhibitor drugs.
### Adverse Effects
* **Common:** Nausea, vomiting, dizziness, drowsiness, headache.
* **Less Common:** Rash, confusion, respiratory depression (especially at high doses or in combination with other CNS depressants).
### Key Drug Interactions
* **MAOIs and other serotonergic agents:** Increased risk of serotonin syndrome.
* **CNS Depressants (e.g., alcohol, benzodiazepines, opioids):** Increased risk of sedation and respiratory depression.
* **CYP2D6 inhibitors (e.g., fluoxetine, paroxetine, quinidine):** May increase dextromethorphan plasma concentrations, increasing the risk of toxicity.
* **Nitrosamine Inhibitor Drugs:** Contraindicated.
### Monitoring
* Monitor for effectiveness in reducing cough and congestion.
* Monitor for signs of adverse effects, particularly CNS depression and serotonin syndrome.
### Clinical Pearls
* TUSQ-QD is for symptomatic relief only and does not treat the underlying cause of the cough.
* Adequate hydration is important to help thin mucus.
* Advise patients to avoid driving or operating heavy machinery until they know how this medication affects them.
* Consult a physician if cough persists for more than 7 days, recurs, or is accompanied by fever, rash, or persistent headache.
