## TUSQ-QD
### Overview
TUSQ-QD is a combination medication containing hydrocodone bitartrate, a Schedule II opioid agonist, and guaifenesin, an expectorant. It is used for the temporary relief of cough and to help loosen phlegm (mucus).
### Primary Indications
* Temporary relief of cough.
* Helps loosen phlegm (mucus) and thin bronchial secretions to make coughs more productive.
### Adult Dosing
* **Typical Dose:** 5 mL or 10 mL orally every 4 to 6 hours as needed.
* **Maximum Dose:** Not to exceed 40 mL in 24 hours.
### Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established for this specific formulation. Consult current pediatric cough and cold guidelines.
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. Dosage reduction may be necessary due to potential for accumulation.
* **Renal Impairment:** Use with caution. Dosage reduction may be necessary.
### Contraindications
* Hypersensitivity to hydrocodone, guaifenesin, or any component of the formulation.
* Acute or severe asthma or respiratory depression.
* Gastrointestinal obstruction or ileus.
* Use in children under 6 years of age (risk of serious respiratory depression).
### Adverse Effects
* **Common:** Drowsiness, dizziness, constipation, nausea, vomiting, dry mouth.
* **Serious:** Respiratory depression, CNS depression, addiction, abuse, misuse, physical dependence, opioid-induced hyperalgesia, severe allergic reactions (anaphylaxis).
### Key Drug Interactions
* **CNS Depressants:** Increased risk of sedation, respiratory depression, profound coma, and death (e.g., alcohol, benzodiazepines, other opioids, sedatives, hypnotics).
* **MAO Inhibitors:** May potentiate the effects of hydrocodone. Avoid concurrent use and for 14 days after stopping MAO inhibitor.
* **CYP3A4 Inhibitors:** May increase hydrocodone plasma concentrations.
* **CYP3A4 Inducers:** May decrease hydrocodone plasma concentrations.
### Monitoring
* Assess for cough and sputum production.
* Monitor for signs of respiratory depression (e.g., slow or shallow breathing).
* Monitor for signs of CNS depression (e.g., sedation, dizziness).
* Assess for constipation and manage as needed.
* Screen for history of substance abuse.
### Clinical Pearls
* TUSQ-QD contains hydrocodone, a Schedule II controlled substance with a high potential for abuse and addiction. Prescribe with caution.
* Advise patients to avoid driving or operating heavy machinery until they know how this medication affects them.
* Educate patients on the risks of concomitant use with alcohol or other CNS depressants.
* Advise patients to seek medical attention if cough persists for more than 7 days, is accompanied by fever, rash, or persistent headache.
