# Ticagrelor
## Overview
Ticagrelor is a direct-acting, reversible P2Y12 platelet inhibitor. It is indicated to reduce the rate of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI).
## Primary Indications
* Reduction of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS).
* Reduction of thrombotic cardiovascular events in patients with a history of myocardial infarction (MI).
## Adult Dosing
* **Acute Coronary Syndrome (ACS):**
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily.
* Treatment duration is typically 12 months. Consider discontinuation of ticagrelor if clinically appropriate.
* **History of Myocardial Infarction (MI):**
* **Maintenance Dose:** 60 mg orally twice daily.
* Continue treatment as long as the patient is at risk for a thrombotic cardiovascular event.
## Pediatric Dosing
* The safety and efficacy of ticagrelor in pediatric patients have not been established.
## Dose Adjustments
* **Hepatic Impairment:** No dose adjustment necessary for mild to moderate hepatic impairment. Caution is advised in severe hepatic impairment.
* **Renal Impairment:** No dose adjustment necessary for mild to moderate renal impairment. In severe renal impairment, consider the risk and benefit.
## Contraindications
* Active bleeding.
* History of transient ischemic attack (TIA) or stroke.
* Known hypersensitivity to ticagrelor or any of its excipients.
## Adverse Effects
* **Most Common:** Bleeding (major, minor, and other), dyspnea, bradycardia, ventricular pauses, hyperuricemia, and accidental trauma.
* **Serious:** Life-threatening bleeding, severe dyspnea.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Ticagrelor is a substrate of CYP3A4. Concomitant use with strong CYP3A4 inhibitors may increase ticagrelor exposure. Concomitant use with strong CYP3A4 inducers may decrease ticagrelor exposure.
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Use with caution in patients taking narrow therapeutic index CYP3A4 substrates (e.g., certain statins, calcium channel blockers).
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding. Concurrent use should be carefully considered based on individual patient risk assessment.
* **Proton Pump Inhibitors (PPIs):** Some studies suggest potential interactions with omeprazole and esomeprazole that may reduce ticagrelor efficacy. Consider separating administration or using alternative agents if clinically appropriate.
* **Simvastatin/Atorvastatin:** Ticagrelor can increase the exposure of simvastatin and atorvastatin. Limit simvastatin dose to 40 mg daily and atorvastatin dose to 80 mg daily.
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor for dyspnea.
* Monitor for bradycardia.
* Assess adherence to medication regimen.
## Clinical Pearls
* Ticagrelor should be initiated as a loading dose as soon as ACS is diagnosed.
* For patients with ACS, ticagrelor is generally preferred over clopidogrel due to greater efficacy in reducing thrombotic events, despite a slightly higher bleeding risk.
* The 60 mg dose is for long-term maintenance in patients with a history of MI.
* Discontinuation of ticagrelor prematurely increases the risk of thrombotic events.
* Patients may experience dyspnea, which is typically mild and transient and does not require discontinuation of the drug.
