## Ticagrelor
### Overview
Ticagrelor is a direct-acting, reversible P2Y12 receptor antagonist that inhibits platelet aggregation.
### Primary Indications
* Reduction of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI).
* Reduction of new heart attack or stroke, and death from cardiovascular causes in patients with ACS.
### Adult Dosing
* **ACS:** 180 mg orally once as a loading dose, followed by 90 mg orally twice daily.
* **Post-MI (secondary prevention):** 60 mg orally twice daily after at least 12 months of dual antiplatelet therapy (DAPT) with a P2Y12 inhibitor and aspirin following ACS, or in patients with a history of MI treated with DAPT.
### Pediatric Dosing
Dosing for pediatric patients is not established.
### Dose Adjustments
No dose adjustment is required for age, sex, weight, or renal impairment.
### Contraindications
* Active pathological bleeding.
* History of intracranial hemorrhage.
* Known hypersensitivity to ticagrelor or any of its components.
### Adverse Effects
* **Major Bleeding:** Gastrointestinal, intracranial, and retroperitoneal bleeding.
* **Dyspnea:** Transient, often resolves with continued therapy.
* **Bradycardia:** Symptomatic and asymptomatic.
* **Hyperuricemia and Gout:** Due to inhibition of URAT1.
* **Ventricular pauses, syncope.**
### Key Drug Interactions
* **CYP3A4 Substrates (e.g., simvastatin, atorvastatin, cyclosporine):** Ticagrelor is a moderate inhibitor of CYP3A4, increasing concentrations of these drugs. Avoid concurrent use of strong CYP3A4 inhibitors.
* **Aspirin:** Concurrent use is recommended, but can increase bleeding risk. Aspirin dose greater than 100 mg may reduce ticagrelor's antiplatelet effect.
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease ticagrelor concentrations.
* **Other Anticoagulants and Antiplatelets:** Increased risk of bleeding.
### Monitoring
* **Bleeding:** Closely monitor for signs and symptoms of bleeding.
* **Renal function:** Although no dose adjustment is needed, monitor as clinically indicated.
* **Uric acid levels:** Monitor periodically, especially in patients with a history of gout.
### Clinical Pearls
* Ticagrelor is a reversible P2Y12 inhibitor, allowing for more rapid return of platelet function compared to clopidogrel if needed.
* The 60 mg twice-daily dose is approved for long-term secondary prevention in specific patient populations.
* If a dose is missed, the patient should take it as soon as they remember and then continue to take it at the next scheduled time. Do not double the dose.
* Management of bleeding requires discontinuation of ticagrelor, and potentially reversing agents if other anticoagulants are involved.
