# Ticagrelor
## Overview
Ticagrelor is an oral, direct-acting, reversible P2Y12 platelet inhibitor. It is part of a class of drugs known as thienopyridines.
## Primary Indications
* Reduction of thrombotic cardiovascular events in patients with Acute Coronary Syndrome (ACS), including those treated with percutaneous coronary intervention (PCI).
* Prevention of thrombotic cardiovascular events in patients with a history of myocardial infarction (MI).
## Adult Dosing
* **ACS:** Loading dose of 180 mg orally once, followed by 90 mg orally twice daily.
* **Post-MI (history of MI):** 60 mg orally twice daily.
* **Duration of therapy:** Typically 12 months for ACS unless contraindications exist. For post-MI, duration is guided by clinical assessment.
## Pediatric Dosing
There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution in patients with severe hepatic impairment due to lack of clinical data. No dose adjustment is recommended for mild to moderate hepatic impairment.
* **Renal Impairment:** No dose adjustment is recommended for mild, moderate, or severe renal impairment.
## Contraindications
* Active bleeding.
* History of intracerebral hemorrhage.
* Known hypersensitivity to ticagrelor or any of its excipients.
## Adverse Effects
* **Most Common:** Dyspnea, bradycardia, bleeding (including major and minor).
* **Serious:** Major bleeding, symptomatic bradycardia, ventricular pauses, transient ischemic attack (TIA), stroke.
* **Other:** Hypertension, chest pain, atrial fibrillation, syncope, thrombocytopenia, contusion.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase ticagrelor exposure.
* **CYP3A4 Substrates:** Ticagrelor may increase plasma concentrations of drugs metabolized by CYP3A4 (e.g., simvastatin, atorvastatin, cyclosporine).
* **Other Antiplatelet Agents/Anticoagulants:** Increased risk of bleeding. Use with caution and monitor closely.
* **Aspirin:** Coadministration is recommended in ACS, but higher maintenance doses of aspirin (>$100 mg) may reduce ticagrelor's efficacy.
* **Simvastatin/Atorvastatin:** Limit simvastatin to 40 mg daily and atorvastatin to 80 mg daily.
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor for signs and symptoms of bradycardia (e.g., dizziness, syncope).
* Assess for adherence and potential drug interactions.
## Clinical Pearls
* Ticagrelor's onset of action is rapid.
* Unlike clopidogrel, ticagrelor does not require metabolic activation.
* Patients experiencing dyspnea should be evaluated to rule out other causes before discontinuing ticagrelor.
* Discontinuation of ticagrelor, especially early, increases the risk of thrombotic events.
