# Ticagrelor
## Overview
Ticagrelor is a direct-acting, reversibly binding P2Y12 platelet inhibitor.
## Primary Indications
* Reduction of thrombotic cardiovascular events in patients with a history of myocardial infarction (MI).
* In combination with aspirin, for the treatment of acute coronary syndrome (ACS) or in patients with a history of MI.
## Adult Dosing
* **Loading Dose:** 180 mg orally once, followed by maintenance dose.
* **Maintenance Dose:** 90 mg orally twice daily for 12 months in ACS patients. For patients with a history of MI, the dose can be 60 mg orally twice daily.
* Dosing duration varies based on clinical indication and patient risk factors; consult current guidelines.
## Pediatric Dosing
* Ticagrelor is not approved for use in pediatric patients.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment needed for mild to moderate renal impairment. Data is limited for severe renal impairment; use with caution.
* **Hepatic Impairment:** No dose adjustment needed for mild to moderate hepatic impairment. Avoid use in severe hepatic impairment.
## Contraindications
* Active pathological bleeding (e.g., intracranial hemorrhage, GI bleeding).
* History of transient ischemic attack (TIA) or stroke.
* Concomitant use with strong CYP3A4 inhibitors.
* Known hypersensitivity to ticagrelor or any of its components.
## Adverse Effects

* **Serious:** Life-threatening bleeding, severe allergic reactions.
## Key Drug Interactions
* **CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin):** Increase ticagrelor exposure; avoid concomitant use.
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine):** Decrease ticagrelor exposure; use with caution.
* **Aspirin:** Concomitant use is common and indicated in ACS, but increases bleeding risk. Specific dose recommendations for aspirin vary by indication and protocol.
* **Other Antiplatelets/Anticoagulants:** Increase bleeding risk; use with caution and monitor closely.
* **Statins:** Ticagrelor can increase simvastatin and atorvastatin exposure; use lower statin doses.
* **Digoxin:** Ticagrelor can increase digoxin exposure; monitor digoxin levels.
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor for dyspnea.
* Inquire about potential drug interactions.

## Clinical Pearls
* Ticagrelor should be taken for the duration recommended by guidelines, typically 12 months for ACS, unless contraindicated.
* Aspirin dose should generally not exceed 100 mg daily for patients taking ticagrelor for secondary prevention, unless there is a specific contraindication to aspirin.
* Patients should be advised to inform their healthcare provider if they experience bleeding, dyspnea, or dizziness.
