# Ticagrelor
## Overview
Ticagrelor is a direct-acting, reversible P2Y12 receptor antagonist that inhibits platelet activation and aggregation.
## Primary Indications
* Reduction of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI).
* In combination with aspirin, for the prevention of atherothrombotic events in patients with a history of MI.
## Adult Dosing
* **ACS:** A 300 mg loading dose should be administered as soon as possible, followed by 90 mg twice daily for at least 12 months. If tolerated, maintenance therapy can be continued at 60 mg twice daily.
* **History of MI:** For patients with a history of MI and at least one other risk factor for occlusive vascular events, the recommended maintenance dose is 60 mg twice daily.
* **Note:** Dosing duration and specific indications may vary based on patient risk factors and local protocols.
## Pediatric Dosing
* There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
* No specific dose adjustments are recommended for renal or hepatic impairment. However, caution is advised in patients with severe hepatic impairment.
## Contraindications
* Active pathological bleeding.
* History of intracranial hemorrhage.
* Known hypersensitivity to ticagrelor or any component of the formulation.
## Adverse Effects
* **Most Common:** Dyspnea, bleeding (major and minor), bradycardia.
* **Serious:** Life-threatening bleeding, ventricular pauses, increased serum creatinine, increased uric acid.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) can increase ticagrelor exposure. Strong CYP3A4 inducers (e.g., rifampin, phenytoin) can decrease exposure.
* **Aspirin:** Ticagrelor is typically used in combination with aspirin, but increased bleeding risk should be monitored.
* **Other Antithrombotics:** Concomitant use with other agents that increase bleeding risk (e.g., heparin, warfarin, GP IIb/IIIa inhibitors) should be done with caution.
* **Simvastatin/Atorvastatin:** Ticagrelor can increase the plasma concentrations of simvastatin and atorvastatin.
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor for bradycardia.
* Regularly assess renal and hepatic function.
* Consider monitoring uric acid levels.
## Clinical Pearls
* Ticagrelor is a P2Y12 inhibitor that requires twice-daily dosing, unlike clopidogrel and prasugrel.
* The 60 mg dose is indicated for long-term maintenance therapy in specific patient populations.
* Aspirin maintenance dose should not exceed 100 mg when used concomitantly with ticagrelor.
* Discontinuation of ticagrelor may increase the risk of thrombotic events; consider the risks and benefits of early discontinuation.
