# Ticagrelor (Brilinta)
## Overview
Ticagrelor is a direct-acting, reversible P2Y12 platelet inhibitor used to reduce the rate of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI).
## Primary Indications
* **Acute Coronary Syndrome (ACS):** To reduce the rate of thrombotic cardiovascular events (including stent thrombosis) in patients with ACS.
* **History of Myocardial Infarction (MI):** In patients with a history of MI (at least 1 year prior) to reduce the rate of cardiovascular death, MI, and stroke.
## Adult Dosing
* **ACS:**
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily for at least 12 months. After 12 months, consideration for extending treatment may be given.
* **Alternative maintenance dose (for patients with history of MI):** 60 mg orally twice daily. This dose is generally considered for long-term secondary prevention after at least 1 year of treatment with 90 mg twice daily.
* **History of MI (at least 1 year prior):**
* **Maintenance Dose:** 60 mg orally twice daily.
## Pediatric Dosing
* There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is required in patients with mild, moderate, or severe renal impairment.
* **Hepatic Impairment:** No dose adjustment is required in patients with mild to moderate hepatic impairment. Ticagrelor is contraindicated in severe hepatic impairment.
## Contraindications
* Hypersensitivity to ticagrelor or any of its components.
* Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage).
* Severe hepatic impairment.
## Adverse Effects
* **Most Common:** Bleeding (including fatal and life-threatening), dyspnea, bradyarrhythmias, non-fatal myocardial infarction.
* **Other Significant:** Ventricular pauses, hyperuricemia, hypotension, increased serum creatinine.
## Key Drug Interactions
* **CYP3A4 Substrates (especially strong inhibitors/inducers):** Ticagrelor is a moderate inhibitor of CYP3A4. Coadministration with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase ticagrelor concentrations. Coadministration with strong CYP3A4 inducers (e.g., rifampin, phenytoin) may decrease ticagrelor concentrations.
* **Other Antiplatelet Agents/Anticoagulants:** Increased risk of bleeding. Concurrent use requires careful monitoring.
* **Statins:** Ticagrelor can increase statin concentrations via CYP3A4 inhibition. Monitor for statin-related adverse effects.
* **Proton Pump Inhibitors (PPIs):** Some data suggest potential reduced efficacy of ticagrelor with concomitant use of omeprazole or esomeprazole, particularly those that are strong CYP2C19 inhibitors. The clinical significance is uncertain.
## Monitoring
* **Bleeding:** Monitor for signs and symptoms of bleeding.
* **Cardiac Rhythm:** Monitor for bradyarrhythmias, particularly in patients with risk factors.
* **Renal and Hepatic Function:** Periodically monitor in patients with pre-existing impairment.
* **Uric Acid Levels:** Monitor periodically, especially in patients with a history of gout or hyperuricemia.
## Clinical Pearls
* Ticagrelor is generally dosed without regard to food.
* Discontinue aspirin in patients receiving ticagrelor for ACS, except for patients with a history of MI where it is indicated. Aspirin is recommended as part of dual antiplatelet therapy (DAPT) in ACS and can be continued for secondary prevention.
* The 180 mg loading dose in ACS is crucial for rapid platelet inhibition.
* Patients experiencing dyspnea should be evaluated for underlying causes, but ticagrelor-induced dyspnea is common and often resolves spontaneously.
* Discontinuation of ticagrelor should be avoided if possible, especially within the first year of ACS, due to increased risk of thrombotic events. If interruption is necessary, consider the risks and benefits.
