# Ticagrelor
## Overview
Ticagrelor is an oral, direct-acting, and reversibly binding P2Y12 platelet inhibitor.
## Primary Indications
* Reduction of thrombotic cardiovascular events (including stent thrombosis) in patients with a history of myocardial infarction (MI) or with acute coronary syndrome (ACS).
## Adult Dosing
* **Acute Coronary Syndrome (ACS):** 180 mg orally once on day 1, followed by 90 mg orally twice daily.
* **History of Myocardial Infarction (MI):** 90 mg orally twice daily.
* **Duration:** Generally continued for at least 12 months, or longer if clinically indicated. May be extended beyond 12 months in certain high-risk patients.
## Pediatric Dosing
* Data is insufficient to recommend use in pediatric patients.
## Dose Adjustments
* No dose adjustment is required for patients with hepatic impairment.
* No dose adjustment is required for patients with renal impairment, although caution may be warranted in severe renal impairment.
## Contraindications
* Active pathological bleeding (e.g., intracranial hemorrhage, gastrointestinal bleeding).
* History of intracranial hemorrhage.
* Hypersensitivity to ticagrelor or any of its components.
## Adverse Effects
* **Common:** Dyspnea, bruising, accidental trauma, bleeding (major and minor), bradycardia.
* **Serious:** Life-threatening bleeding, ventricular pauses, increased serum creatinine, increased uric acid levels, cough.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Ticagrelor is a substrate of CYP3A4. Coadministration with strong CYP3A4 inhibitors may increase ticagrelor exposure. Coadministration with strong CYP3A4 inducers may decrease ticagrelor exposure.
* **CYP3A4 Substrates:** Ticagrelor can inhibit CYP3A4, increasing the exposure of concomitant CYP3A4 substrates (e.g., simvastatin, atorvastatin). Limit doses of statins metabolized by CYP3A4 (e.g., simvastatin, atorvastatin) to 40 mg daily.
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding. Use with caution and consider the cumulative risk.
* **Proton Pump Inhibitors (PPIs):** Some studies suggest PPIs may reduce ticagrelor efficacy. Consider the benefits and risks, and if a PPI is necessary, consider separating doses or using agents with less CYP3A4 interaction potential (e.g., famotidine).
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor renal function and uric acid levels, particularly in patients with pre-existing renal impairment or hyperuricemia.
* Monitor heart rate for bradycardia.
## Clinical Pearls
* Ticagrelor is dosed twice daily. Patients should be educated on the importance of adherence to this dosing schedule.
* The management of bleeding in patients on ticagrelor requires careful consideration of the risks of discontinuing the antiplatelet therapy.
* Discontinuation of ticagrelor should be avoided if possible, as it can increase the risk of thrombotic events.
