# Ticagrelor
## Overview
Ticagrelor is a direct-acting, reversible P2Y12 receptor antagonist used to prevent thrombotic events.
## Primary Indications
* In combination with aspirin for the prevention of thrombotic events (stroke, myocardial infarction) in patients with acute coronary syndrome (ACS).
* In combination with aspirin for the prevention of thrombotic events in patients with a history of myocardial infarction (MI) and at least one additional risk factor for occlusive vascular events.
## Adult Dosing
* **Acute Coronary Syndrome (ACS):**
* Loading Dose: 180 mg orally once.
* Maintenance Dose: 90 mg orally twice daily. Continue for at least 12 months, or longer based on individual risk assessment and bleeding risk.
* **Post-Myocardial Infarction (MI) with risk factors:**
* 90 mg orally twice daily. Continue for at least 12 months.
## Pediatric Dosing
There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
* **Hepatic Impairment:**
* Moderate hepatic impairment: Use with caution.
* Severe hepatic impairment: Contraindicated.
* **Renal Impairment:** No dose adjustment necessary for mild, moderate, or severe renal impairment.
## Contraindications
* Active bleeding.
* History of intracerebral hemorrhage.
* Hypersensitivity to ticagrelor or any component of the formulation.
* Severe hepatic impairment.
## Adverse Effects
* **Common:** Dyspnea, bleeding (including life-threatening), bradycardia.
* **Serious:** Major bleeding, ventricular pauses, hypersensitivity reactions.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Ticagrelor is a substrate of CYP3A4. Co-administration with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase ticagrelor concentrations and risk of bleeding. Co-administration with strong CYP3A4 inducers (e.g., rifampin, carbamazepine) may decrease ticagrelor concentrations.
* **Aspirin:** Concurrent use is indicated for ACS and post-MI patients.
* **Anticoagulants (e.g., warfarin, DOACs):** Increased risk of bleeding. Use with caution.
* **Statins:** Ticagrelor can increase simvastatin and atorvastatin concentrations. Avoid simvastatin >40 mg and atorvastatin >80 mg.
* **Omeprazole/Esomeprazole:** May reduce ticagrelor exposure. Consider separating administration times if concurrent use is necessary.
## Monitoring
* **Bleeding:** Monitor for signs and symptoms of bleeding, especially in patients with increased risk factors.
* **ECG:** Monitor for bradyarrhythmias.
## Clinical Pearls
* Ticagrelor's reversibility means its antiplatelet effect diminishes more rapidly than clopidogrel after discontinuation, which may be beneficial if urgent surgery is required.
* Patients may experience dyspnea, which is often transient and not associated with pulmonary disease.
* The maintenance dose of 90 mg twice daily is generally for the first year in ACS, with a potential switch to 60 mg twice daily for longer-term maintenance based on specific guidelines and patient risk assessment (consult current guidelines for this specific indication).
