# Ticagrelor (Brilinta)
## Overview
Ticagrelor is an orally administered, direct-acting P2Y12 receptor antagonist that is reversible. It is used to reduce the rate of thrombotic cardiovascular events in patients with a history of myocardial infarction (MI) or acute coronary syndrome (ACS).
## Primary Indications
* Prevention of thrombotic cardiovascular events in patients with ACS (unstable angina, NSTEMI, STEMI).
* Prevention of thrombotic cardiovascular events in patients with a history of MI.
## Adult Dosing
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily for at least 12 months, or until recommended by a physician.
* **For patients with a history of MI (≥ 1 year):** 60 mg orally twice daily.
## Pediatric Dosing
Dosing for pediatric patients has not been established.
## Dose Adjustments
* **Hepatic Impairment:** No dose adjustment is recommended for patients with mild to moderate hepatic impairment. Ticagrelor has not been studied in severe hepatic impairment and should be used with caution.
* **Renal Impairment:** No dose adjustment is recommended for patients with mild to severe renal impairment.
## Contraindications
* Active pathological bleeding.
* History of transient ischemic attack (TIA) or stroke.
* Known hypersensitivity to ticagrelor or any of its excipients.
## Adverse Effects
* **Most Common:** Dyspnea, bruising, bleeding (including spontaneous or procedure-related).
* **Serious:** Major bleeding, including intracranial and fatal bleeding. Ventricular pauses.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Avoid concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) and consider alternatives for strong CYP3A4 substrates with a narrow therapeutic index (e.g., simvastatin, atorvastatin).
* **CYP3A4 Inducers:** Concomitant use with strong CYP3A4 inducers (e.g., rifampin, phenytoin) may decrease ticagrelor exposure and efficacy.
* **Aspirin:** While generally co-administered, dose adjustments of aspirin may be considered. High-dose aspirin may reduce ticagrelor's efficacy.
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding. Use with caution and monitor closely.
## Monitoring
* **Bleeding:** Monitor for any signs or symptoms of bleeding.
* **ECG:** Transient ventricular pauses may occur, usually without symptoms.
## Clinical Pearls
* Ticagrelor is generally preferred over clopidogrel in ACS patients due to superior efficacy in reducing ischemic events, despite a similar or slightly increased bleeding risk.
* It is crucial to ensure patients continue therapy for the recommended duration, as stopping prematurely can increase the risk of thrombotic events.
* Discontinue ticagrelor at least 5 days before elective surgery if a lower bleeding risk is desired.
