# Ticagrelor (Brilinta)
## Overview
Ticagrelor is a direct-acting, reversibly binding P2Y12 receptor antagonist. It inhibits adenosine diphosphate (ADP)-induced platelet aggregation.
## Primary Indications
* **Acute Coronary Syndrome (ACS):** In combination with aspirin, for the reduction of thrombotic cardiovascular events in patients with a history of ACS.
* **Myocardial Infarction (MI):** In patients with a history of MI, ticagrelor can be used with aspirin to reduce the rate of cardiovascular death, myocardial infarction, and stroke.
## Adult Dosing
* **ACS:**
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily for up to 12 months. Some patients may benefit from treatment beyond 12 months; decisions should be individualized.
* **History of MI:**
* **Maintenance Dose:** 60 mg orally twice daily. This dose is used in patients with a history of MI who are currently receiving or have previously received aspirin and are at high risk for thrombotic events.
## Pediatric Dosing
Ticagrelor is not recommended for use in pediatric patients. Safety and efficacy have not been established.
## Dose Adjustments
* **Hepatic Impairment:** No dose adjustment is required in patients with mild to moderate hepatic impairment. Caution is advised in severe hepatic impairment.
* **Renal Impairment:** No dose adjustment is required in patients with mild to moderate renal impairment. In patients with severe renal impairment or end-stage renal disease (ESRD), the 60 mg dose twice daily may be considered if the benefits outweigh the risks, but caution is advised.
## Contraindications
* Documented hypersensitivity to ticagrelor or any of its excipients.
* Active pathological bleeding.
## Adverse Effects
* **Common:** Bleeding (major, minor, gastrointestinal), dyspnea, bradycardia, ventricular pauses.
* **Serious:** Life-threatening bleeding, anaphylaxis.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase ticagrelor concentrations. Concomitant use with strong CYP3A4 inducers (e.g., rifampin, carbamazepine) may decrease ticagrelor concentrations.
* **CYP3A4 Substrates (with narrow therapeutic index):** Avoid concomitant use of ticagrelor with sensitive CYP3A4 substrates like simvastatin or lovastatin at doses >40 mg.
* **Aspirin:** Continued aspirin use is generally recommended, but it increases the risk of bleeding.
* **Other Antiplatelet Agents/Anticoagulants:** Use with caution due to increased risk of bleeding.
## Monitoring
* **Bleeding:** Monitor for signs and symptoms of bleeding.
* **Cardiac Rhythm:** Assess for bradycardia and ventricular pauses, especially in patients with risk factors.
* **Renal and Hepatic Function:** Monitor periodically, particularly in patients with severe impairment.
## Clinical Pearls
* Ticagrelor is a reversible P2Y12 inhibitor, allowing for quicker return of platelet function compared to clopidogrel, which can be beneficial if urgent surgery is needed.
* The 90 mg twice-daily dose is used for ACS patients, while the 60 mg twice-daily dose is for patients with a history of MI.
* Dyspnea is a common side effect and may not be related to underlying cardiac or pulmonary conditions.
* Avoid concomitant use with CYP3A4 substrates that have a narrow therapeutic index at higher doses.
