# Ticagrelor (Brilinta)
## Overview
Ticagrelor is an orally active, direct-acting P2Y12 receptor antagonist. It is a member of the cyclopentyltriazolopyrimidine (CPTP) class and is reversible.
## Primary Indications
* **Acute Coronary Syndrome (ACS):** In combination with aspirin, for the prevention of thrombotic events in patients with ACS, including unstable angina, non-ST-elevation myocardial infarction (NSTEMI), and ST-elevation myocardial infarction (STEMI).
* **Myocardial Infarction (MI):** In patients with a history of MI, to reduce the rate of cardiovascular death, MI, and stroke.
## Adult Dosing
* **ACS:**
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily.
* **History of MI:** 60 mg orally twice daily.
* **Concomitant Medications:** Ticagrelor should be used in combination with aspirin. The recommended maintenance dose of aspirin is typically 75-100 mg daily.
## Pediatric Dosing
The use of ticagrelor in pediatric patients is not established.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment. However, caution is advised in severe hepatic impairment.
## Contraindications
* Active pathological bleeding.
* History of intracranial hemorrhage.
* Hypersensitivity to ticagrelor or any component of the formulation.
## Adverse Effects
* **Bleeding:** The most common and serious adverse effect. Includes spontaneous bleeding, surgical bleeding, and other bleeding events.
* **Dyspnea:** May occur and is generally mild and transient.
* **Bradycardia:** Symptomatic bradycardia can occur.
* **Ventricular pauses:** Transient pauses have been reported.
* **Hyperuricemia and Gout:** Can occur.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Ticagrelor is a substrate of CYP3A4. Potent inhibitors (e.g., ketoconazole, ritonavir) can increase ticagrelor exposure. Potent inducers (e.g., rifampin, carbamazepine) can decrease exposure.
* **CYP3A4 Substrates:** Ticagrelor can increase the plasma concentration of CYP3A4 substrates (e.g., simvastatin, atorvastatin).
* **Other Antithrombotics:** Concomitant use with other antithrombotics (e.g., warfarin, other P2Y12 inhibitors, NSAIDs) increases the risk of bleeding.
* **Digoxin:** Ticagrelor can increase digoxin levels.
## Monitoring
* **Bleeding:** Closely monitor for signs and symptoms of bleeding.
* **Efficacy:** Assess for prevention of thrombotic events as per indication.
* **Bradycardia:** Monitor heart rate and rhythm, especially in patients with pre-existing risk factors for bradycardia.
* **Renal and Hepatic Function:** Baseline and periodic monitoring may be considered, particularly in patients with severe impairment.
## Clinical Pearls
* Ticagrelor is a reversible P2Y12 inhibitor, differentiating it from clopidogrel and prasugrel.
* The loading dose is crucial for rapid platelet inhibition in ACS.
* For ACS patients, ticagrelor 90 mg BID is used. For post-MI patients without a recent ACS, ticagrelor 60 mg BID is used.
* Discontinuation of ticagrelor prematurely increases the risk of thrombotic events.
* Be aware of the increased risk of bleeding with concomitant use of other antithrombotics and CYP3A4 inhibitors.
* Consider the potential for drug interactions with CYP3A4 substrates and digoxin.
