# Ticagrelor (Brilinta)
## Overview
Ticagrelor is a direct-acting, reversibly binding P2Y12 receptor antagonist.
## Primary Indications
* Inhibition of platelet aggregation in patients with a history of myocardial infarction (MI) or acute coronary syndrome (ACS).
* Reduction of thrombotic cardiovascular events (including stent thrombosis) in patients treated with aspirin for ACS.
## Adult Dosing
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily for the first 12 months. After 12 months, consider 60 mg orally twice daily for secondary prevention based on individual risk assessment.
* **Following ACS:** Dual antiplatelet therapy (DAPT) with aspirin is standard.
## Pediatric Dosing
There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is required.
* **Hepatic Impairment:** No dose adjustment is required.
## Contraindications
* Active bleeding.
* History of intracranial hemorrhage.
* Hypersensitivity to ticagrelor or any component of the formulation.
## Adverse Effects
* **Common:** Dyspnea, bleeding (major and minor), bradycardia.
* **Serious:** Major bleeding, clinically significant bradycardia, ventricular pauses, respiratory impairment.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin) may increase ticagrelor exposure. Concomitant use with strong CYP3A4 inducers (e.g., rifampin, phenytoin, carbamazepine) may decrease ticagrelor exposure.
* **CYP3A4 Substrates (e.g., simvastatin, atorvastatin):** Ticagrelor can increase the exposure of these drugs. Dose reductions of simvastatin and atorvastatin may be necessary.
* **Aspirin:** Increased risk of bleeding.
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding.
## Monitoring
* **Bleeding:** Monitor for signs and symptoms of bleeding.
* **Heart Rate:** Monitor for bradycardia, especially in patients with pre-existing bradycardic rhythm or heart failure.
## Clinical Pearls
* Ticagrelor is a P2Y12 inhibitor that is reversibly bound, allowing for rapid platelet function recovery upon discontinuation, which may be advantageous in patients requiring urgent surgery.
* The maintenance dose may be reduced to 60 mg twice daily after 12 months of DAPT in certain patient populations, but this decision requires careful risk-benefit assessment.
* Patients experiencing dyspnea should be monitored closely, but ticagrelor should not be discontinued solely based on this symptom unless it is severe or persistent.
