# Ticagrelor
## Overview
Ticagrelor is a direct-acting, reversible P2Y12 platelet inhibitor.
## Primary Indications
* Reduction of thrombotic cardiovascular events (e.g., myocardial infarction, stroke) in patients with acute coronary syndrome (ACS) or a history of myocardial infarction.
## Adult Dosing
* **ACS:** Loading dose of 180 mg orally once, followed by 90 mg orally twice daily.
* **History of MI:** 60 mg orally twice daily.
## Pediatric Dosing
* Dosing for pediatric patients is not established.
## Dose Adjustments
* No dose adjustment is necessary for patients with hepatic impairment.
* No dose adjustment is necessary for patients with renal impairment.
## Contraindications
* Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, voriconazole, ritonavir, clarithromycin).
* History of hypersensitivity to ticagrelor or any component of the formulation.
## Adverse Effects
* **Common:** Dyspnea, bradycardia, bleeding (major and minor).
* **Serious:** Severe bleeding, ventricular pauses, hyperkalemia, angioedema.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor inhibits CYP3A4. Coadministration with sensitive CYP3A4 substrates (e.g., statins, calcium channel blockers) may increase exposure to these drugs.
* **CYP3A4 Inducers:** Inducers of CYP3A4 (e.g., rifampin, carbamazepine, phenytoin) may decrease ticagrelor exposure and efficacy.
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding.
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor potassium levels, especially in patients with renal or hepatic impairment or those taking other medications that increase potassium.
* Monitor heart rate for bradycardia or ventricular pauses.
## Clinical Pearls
* Ticagrelor has a faster onset and offset of action compared to clopidogrel.
* Dyspnea is a common side effect and may require discontinuation, but it is not necessarily indicative of a cardiac issue.
* Ticagrelor should generally be taken for at least 12 months in ACS patients, unless contraindicated.
* Discontinuation of ticagrelor before the recommended duration increases the risk of thrombotic events.
