# Ticagrelor (Brilinta)
## Overview
Ticagrelor is an oral P2Y12 platelet inhibitor used to reduce the risk of thrombotic cardiovascular events. It is a direct-acting, reversible antagonist.
## Primary Indications
* Reduction of thrombotic cardiovascular events (e.g., myocardial infarction, stroke) in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI) who are treated with aspirin.
## Adult Dosing
* **ACS:** Loading dose of 180 mg orally once, followed by 90 mg orally twice daily.
* **Post-MI (at least 1 year after ACS or with risk factors):** 60 mg orally twice daily.
* **Duration of therapy:** Typically 12 months in ACS, but may be extended based on individual risk assessment. For post-MI patients, therapy duration is not well established beyond 1 year.
## Pediatric Dosing
* Dosing for pediatric patients is not established.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is recommended for mild to severe renal impairment.
* **Hepatic Impairment:** No dose adjustment is recommended for mild to moderate hepatic impairment. Use with caution in severe hepatic impairment.
## Contraindications
* Active pathological bleeding (e.g., intracranial hemorrhage, active gastrointestinal bleeding).
* History of transient ischemic attack (TIA) or stroke.
* Concomitant use of strong CYP3A4 inhibitors.
* Known hypersensitivity to ticagrelor or any of its excipients.
## Adverse Effects
* **Most common:** Bleeding (including fatal and serious bleeding), dyspnea, bradyarrhythmia.
* **Other:** Ventricular pauses, hyperkalemia, increased serum creatinine.
## Key Drug Interactions
* **CYP3A4 Substrates (especially strong inhibitors):** Ticagrelor is a moderate inhibitor of CYP3A4. Avoid concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, voriconazole, clarithromycin, ritonavir).
* **CYP3A4 Substrates (especially statins):** Ticagrelor can increase plasma concentrations of statins metabolized by CYP3A4 (e.g., simvastatin, atorvastatin). Use caution and consider lower statin doses.
* **CYP2C19 Substrates:** Ticagrelor is a moderate inhibitor of CYP2C19, which can affect clopidogrel metabolism. Avoid coadministration with clopidogrel.
* **Aspirin:** Concomitant use increases bleeding risk.
* **Anticoagulants (e.g., warfarin, heparin, DOACs):** Concomitant use increases bleeding risk.
* **Non-steroidal anti-inflammatory drugs (NSAIDs):** Increased risk of gastrointestinal bleeding.
## Monitoring
* **Bleeding:** Monitor for signs and symptoms of bleeding.
* **Renal function:** Monitor serum creatinine.
* **Electrolytes:** Monitor potassium levels, especially in patients with risk factors for hyperkalemia.
* **ECG:** Monitor for bradyarrhythmias.
## Clinical Pearls
* Ticagrelor should be taken for the duration recommended by the treating physician, typically 12 months for ACS patients.
* Discontinuation before the recommended duration can increase the risk of thrombotic events.
* Patients experiencing dyspnea should be evaluated for underlying causes, but if no other cause is found, ticagrelor may be continued if benefits outweigh risks.
* Dose of aspirin should be maintained at 75-100 mg once daily in ACS patients.
