# Ticagrelor (Brilinta)
## Overview
Ticagrelor is an oral, direct-acting, and reversible P2Y12 receptor antagonist. It is used to reduce the rate of cardiovascular events in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI).
## Primary Indications
* **Acute Coronary Syndrome (ACS):** In combination with aspirin, to reduce the rate of thrombotic cardiovascular events in patients with ACS (unstable angina, non-ST-elevation MI, or ST-elevation MI).
* **History of Myocardial Infarction (MI):** In patients with a history of MI at high risk for occlusive thrombotic events.
## Adult Dosing
* **ACS:**
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily.
* **Duration:** For ACS, generally continued for at least 12 months, or longer if clinically indicated.
* **History of MI:**
* **Maintenance Dose:** 60 mg orally twice daily.
* **Duration:** In patients with a history of MI, ticagrelor 60 mg twice daily should be used for at least 12 months, provided the risk of ischemic events outweighs the risk of bleeding.
## Pediatric Dosing
Dosing for pediatric patients has not been established.
## Dose Adjustments
* **Hepatic Impairment:** No dose adjustment is recommended for patients with mild to moderate hepatic impairment. Use with caution in severe hepatic impairment.
* **Renal Impairment:** No dose adjustment is recommended for patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
## Contraindications
* Active bleeding.
* History of transient ischemic attack (TIA) or stroke.
* Known hypersensitivity to ticagrelor or any of its excipients.
## Adverse Effects
* **Most Common:** Bleeding (including life-threatening and fatal bleeding), dyspnea, bradycardia.
* **Other:** Nausea, diarrhea, ventricular pauses, cough, rash.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase ticagrelor exposure. Concurrent use with strong CYP3A4 inducers (e.g., rifampin, carbamazepine) may decrease ticagrelor exposure.
* **CYP3A4 Substrates (e.g., simvastatin, atorvastatin):** Avoid concomitant use of ticagrelor with simvastatin or atorvastatin at doses >40 mg due to increased risk of myopathy.
* **Other Antiplatelets/Anticoagulants:** Concomitant use increases the risk of bleeding. Careful monitoring is required.
## Monitoring
* **Bleeding:** Monitor closely for signs and symptoms of bleeding.
* **Heart Rate:** Ticagrelor can cause asymptomatic ventricular pauses. ECG monitoring may be considered in specific situations.
* **Renal and Hepatic Function:** Baseline and periodic assessment may be beneficial, especially in patients with pre-existing impairment or those at risk.
## Clinical Pearls
* Ticagrelor is a reversible P2Y12 inhibitor, unlike clopidogrel and prasugrel.
* It should be taken with or without food.
* Aspirin dose should be limited to 100 mg when used concurrently with ticagrelor to minimize bleeding risk.
* Discontinuation of ticagrelor may increase the risk of thrombotic events. If discontinuation is necessary for surgery, consider the risks and benefits and restart as soon as possible.
