# Ticagrelor (Brilinta)
## Overview
Ticagrelor is an orally administered, direct-acting, reversible P2Y12 platelet inhibitor. It is part of a class of drugs known as antiplatelet agents.
## Primary Indications
* Reduction of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS).
* Reduction of thrombotic cardiovascular events in patients with a history of myocardial infarction (MI) and at least one additional risk factor for cardiovascular events.
## Adult Dosing
* **Acute Coronary Syndrome (ACS):** A loading dose of 180 mg orally once, followed by 90 mg orally twice daily. Treatment duration is typically 12 months.
* **Post-Myocardial Infarction (MI) with risk factors:** 60 mg orally twice daily.
## Pediatric Dosing
There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
No dose adjustments are typically required based on age, sex, or renal impairment. However, caution is advised in patients with severe hepatic impairment.
## Contraindications
* Active pathological bleeding.
* History of intracranial hemorrhage.
* Known hypersensitivity to ticagrelor or any of its components.
## Adverse Effects
* **Common:** Dyspnea, bruising, bleeding (including fatal and serious cases), hypotension.
* **Serious:** Major bleeding, minor bleeding, ventricular pauses, bradycardia.
## Key Drug Interactions
* **CYP3A4 Substrates:** Ticagrelor is a moderate inhibitor of CYP3A4. Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) may increase ticagrelor exposure. Concomitant use with strong CYP3A4 inducers (e.g., rifampin, phenytoin) may decrease ticagrelor exposure.
* **CYP3A4 Substrates (e.g., simvastatin, atorvastatin):** Ticagrelor can increase plasma concentrations of certain statins metabolized by CYP3A4. Dose reduction of the statin may be necessary.
* **Other Antiplatelet Agents/Anticoagulants:** Increased risk of bleeding. Use with caution and consider the duration of each agent.
* **Aspirin:** While often co-administered in ACS, high-dose aspirin may reduce the efficacy of ticagrelor.
## Monitoring
* Monitor for signs and symptoms of bleeding.
* Monitor for dyspnea.
* Consider ECG monitoring for bradycardia, particularly in patients at risk.
## Clinical Pearls
* Ticagrelor's effect is reversible, unlike clopidogrel.
* Patients treated for ACS should continue aspirin indefinitely at a dose determined by local protocol, alongside ticagrelor.
* Discontinuation of ticagrelor increases the risk of thrombotic events. If discontinuation is necessary, reinitiate as soon as possible unless contraindicated.
* Ticagrelor should not be used with potent CYP3A4 inhibitors or inducers without careful consideration and potential dose adjustments.
