# Ticagrelor (Brilinta)
## Overview
Ticagrelor is a direct-acting, reversible P2Y12 platelet inhibitor.
## Primary Indications
* **Acute Coronary Syndrome (ACS):** To reduce the rate of thrombotic cardiovascular events in patients with a history of ACS.
* **Myocardial Infarction (MI):** In patients with a history of MI (≥1 year prior) with at least one risk factor for occlusive vascular events, to reduce the rate of cardiovascular death, MI, and stroke.
## Adult Dosing
* **ACS:**
* **Loading Dose:** 180 mg orally once.
* **Maintenance Dose:** 90 mg orally twice daily for up to 12 months. In select patients (e.g., those with continued high ischemic risk and low bleeding risk), continuation beyond 12 months may be considered.
* **Post-MI (≥1 year prior):**
* **Maintenance Dose:** 60 mg orally twice daily.
## Pediatric Dosing
* There is no established pediatric dosing for ticagrelor.
## Dose Adjustments
* No dose adjustment is required for patients with mild to severe hepatic impairment. Ticagrelor is not recommended in patients with severe hepatic impairment.
* No dose adjustment is required for patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
## Contraindications
* Active pathological bleeding (e.g., intracranial hemorrhage, active gastrointestinal bleeding).
* History of intracranial hemorrhage.
* Known hypersensitivity to ticagrelor or any component of the formulation.
## Adverse Effects
* **Common:** Dyspnea, bradycardia, syncope, bruising, contusions.
* **Serious:** Bleeding (major, minor), ventricular pauses, angioedema, severe skin reactions.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Ticagrelor is a substrate of CYP3A4. Avoid concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) and inducers (e.g., rifampin, carbamazepine).
* **CYP3A4 Substrates:** Ticagrelor can inhibit CYP3A4, increasing the concentration of substrates like simvastatin, atorvastatin, and cyclosporine. Dose adjustments of these agents may be necessary.
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding. Use with caution and monitor closely.
* **Digoxin:** Ticagrelor may increase digoxin concentrations. Monitor digoxin levels and clinical status.
## Monitoring
* **Bleeding:** Closely monitor for signs and symptoms of bleeding.
* **Heart Rate:** Monitor for bradycardia and pauses, especially in patients with risk factors.
* **Renal and Hepatic Function:** Monitor periodically.
## Clinical Pearls
* Ticagrelor is a reversible P2Y12 inhibitor, unlike clopidogrel. This may lead to a more rapid onset and offset of action.
* The 60 mg twice-daily dose is indicated for patients at least 1 year post-MI with at least one risk factor for vascular events.
* Patients who cannot take ticagrelor due to hypersensitivity or contraindications may be considered for alternative antiplatelet therapies.
* Ticagrelor is associated with a higher rate of dyspnea compared to clopidogrel.
