# Ticagrelor (Brilinta)
## Overview
Ticagrelor is a direct-acting, reversibly binding P2Y12 receptor antagonist used to prevent thrombotic events in patients with acute coronary syndrome (ACS) or a history of myocardial infarction (MI).
## Primary Indications
* Treatment of patients with acute coronary syndrome (ACS).
* Secondary prevention of thrombotic events in patients with a history of MI.
## Adult Dosing
* **ACS:** Loading dose of 180 mg orally once, followed by 90 mg orally twice daily.
* **Post-MI (secondary prevention):** 90 mg orally twice daily.
* **Note:** Dosing duration may vary based on clinical factors and physician discretion, often for at least 12 months following ACS.
## Pediatric Dosing
Ticagrelor is not approved for use in pediatric patients.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment necessary.
* **Hepatic Impairment:** Use with caution in severe hepatic impairment. No dose adjustment necessary.
## Contraindications
* Hypersensitivity to ticagrelor or any component of the formulation.
* Active pathological bleeding.
## Adverse Effects
* **Major Bleeding:** This is the most significant risk. Can occur anywhere in the body.
* **Dyspnea:** Commonly reported, often transient and not associated with underlying lung disease.
* **Bradycardia:** Symptomatic bradycardia can occur.
* **Increased serum creatinine:** Typically mild and reversible.
* **Ventricular pauses:** Reported on ECG.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Ticagrelor is a substrate of CYP3A4. Strong inhibitors can increase ticagrelor exposure; strong inducers can decrease it.
* **Strong CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin, ritonavir):** May increase ticagrelor plasma concentration. Consider avoiding concomitant use or reducing ticagrelor dose.
* **Pro-arrhythmic Drugs:** Use with caution.
* **Other Antithrombotics (e.g., anticoagulants, other antiplatelets):** Increased risk of bleeding.
## Monitoring
* **Bleeding:** Closely monitor for any signs and symptoms of bleeding.
* **Renal Function:** Monitor serum creatinine periodically.
* **Electrolytes:** Monitor uric acid and potassium levels periodically.
## Clinical Pearls
* Ticagrelor is a P2Y12 inhibitor that works faster and is more potent than clopidogrel but carries a higher bleeding risk.
* The 180 mg loading dose is crucial for ACS patients to achieve rapid platelet inhibition.
* Do not administer ticagrelor with aspirin doses higher than the recommended maintenance dose (typically 100 mg) for secondary prevention, as this may increase bleeding risk without additional cardiovascular benefit.
* Discontinue ticagrelor at least 5 days prior to elective surgery if possible, but weigh the risk of stent thrombosis against the risk of surgical bleeding.
