## Primary Indications

## Adult Dosing
**Standard Dose:** One tablet (sofosbuvir 400 mg/velpatasvir 100 mg) taken orally once daily.
## Pediatric Dosing
* **12 years of age and older:** One tablet (sofosbuvir 400 mg/velpatasvir 100 mg) taken orally once daily.

## Dose Adjustments

## Contraindications

* **Amiodarone:** Concomitant use with sofosbuvir/velpatasvir can lead to symptomatic bradycardia. Monitor heart rate closely and consider alternatives if possible.
* **Antacids, H2 Receptor Antagonists, Proton Pump Inhibitors:** May decrease velpatasvir exposure. Separate administration by at least 4 hours (PPIs by at least 12 hours).
## Monitoring
* **HCV RNA levels:** To assess treatment efficacy.
* **Liver function tests:** Baseline and periodically during treatment.
* **Hemoglobin/Hematocrit:** For potential anemia.
* **Heart rate:** If co-administered with amiodarone.
* **HBV DNA:** Consider testing for HBV before initiating treatment, especially in patients with risk factors for HBV infection.
## Clinical Pearls
* This medication is typically given for 12 weeks, but duration may be shorter (e.g., 8 weeks) or longer (e.g., 24 weeks) depending on the HCV genotype, prior treatment history, and presence of cirrhosis. Consult specific treatment guidelines.
* It can be taken with or without food.
