# Tabovoihep
## Overview
Tabovoihep is a brand name for a medication containing pegylated interferon alfa-2a. It is a recombinant protein that mimics the body's natural interferon response.
## Primary Indications
* Hepatitis C (in combination with other agents)
* Hepatitis B
## Adult Dosing
* **Hepatitis C:** 180 mcg subcutaneously once weekly for 48 weeks (in combination therapy).
* **Hepatitis B:** 180 mcg subcutaneously once weekly for at least 48 weeks.
## Pediatric Dosing
* **Hepatitis B:** Dosing in pediatric patients is not well-established and should be determined by a specialist.
## Dose Adjustments
* **Hematologic Abnormalities:** Dose reduction may be necessary for neutropenia, thrombocytopenia, or anemia. Refer to specific clinical guidelines for threshold values and reduction schedules.
* **Hepatic Impairment:** Dose reduction may be necessary in patients with decompensated cirrhosis.
## Contraindications
* Hypersensitivity to peginterferon alfa-2a, interferon alfa, or any component of the formulation.

## Adverse Effects

Serious adverse effects can include severe psychiatric disturbances (depression, suicidal ideation), autoimmune disorders, severe hematologic abnormalities, and thyroid dysfunction.
## Key Drug Interactions
* **Theophylline:** Peginterferon alfa-2a may decrease theophylline clearance, requiring monitoring of theophylline levels and potential dose adjustments.
* **Zidovudine:** May increase zidovudine levels, increasing the risk of toxicity.
* **Ribavirin:** Concurrent use with ribavirin (for Hepatitis C) requires careful monitoring due to potential for additive hematologic toxicity (anemia). Specific dosing regimens are critical.
## Monitoring
* Complete blood counts (CBC) with differential and platelet count.
* Liver function tests (ALT, AST, bilirubin).
* Thyroid function tests (TSH).
* Assays for Hepatitis B virus DNA or Hepatitis C virus RNA.
* Psychiatric status.

* Administer subcutaneous injections in the abdomen or thigh.
