# Tabovoihep
## Overview
Tabovoihep is a brand name for a combination product containing tenofovir alafenamide (TAF) and emtricitabine (FTC). It is a nucleotide reverse transcriptase inhibitor (NtRTI) and a nucleoside reverse transcriptase inhibitor (NRTI), respectively.
## Primary Indications
Tabovoihep is indicated for the treatment of human immunodeficiency virus type 1 (HIV-1) infection in adults and pediatric patients.
## Adult Dosing
The recommended dose is one tablet (TAF 25 mg/FTC 200 mg) taken orally once daily.
## Pediatric Dosing
For treatment-naive or treatment-experienced HIV-1 infected pediatric patients weighing at least 25 kg: one tablet (TAF 25 mg/FTC 200 mg) taken orally once daily.
## Dose Adjustments
No dose adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment (CrCl < 30 mL/min) or end-stage renal disease, use is not recommended due to insufficient data. No dose adjustment is required for mild to moderate hepatic impairment. Data for use in severe hepatic impairment are limited.
## Contraindications
None specific to the combination product beyond known hypersensitivity to any of its components.
## Adverse Effects
The most common adverse reactions (incidence >10%) include nausea, diarrhea, headache, abdominal pain, dizziness, fatigue, rash, and insomnia. Less common but serious adverse effects may include lactic acidosis, severe hepatomegaly with steatosis, and immune reconstitution inflammatory syndrome (IRIS).
## Key Drug Interactions
* **Strong CYP3A inhibitors/inducers:** May alter the plasma concentrations of TAF/FTC.
* **Anticonvulsants (e.g., carbamazepine, phenytoin):** May decrease plasma concentrations of TAF/FTC.
* **Rifampin:** May decrease plasma concentrations of TAF/FTC.
* **Herbal products (e.g., St. John's wort):** May decrease plasma concentrations of TAF/FTC.
* **Other antiretrovirals:** Consult specific drug interaction resources.
## Monitoring
* Regular assessment of liver function tests (ALT, AST, bilirubin), renal function (serum creatinine, eGFR), and CD4+ T-cell count.
* Monitor for signs and symptoms of lactic acidosis or liver toxicity.
* Pregnancy testing in women of childbearing potential.
## Clinical Pearls
* Tabovoihep can be taken with or without food.
* It is a component of several recommended HIV-1 treatment regimens.
* For patients who cannot swallow tablets whole, the tablet may be carefully broken in half and the two halves taken immediately.
