# Tabvoihep
## Overview
Tabvoihep is a brand name for an injectable medication containing hepatitis B immune globulin (HBIG). It provides passive immunity against the hepatitis B virus (HBV).
## Primary Indications
* Prophylaxis against HBV infection following exposure (e.g., needle-stick injury, sexual exposure to an HBV-infected individual).
* Prophylaxis in neonates born to HBsAg-positive mothers.
* Post-exposure prophylaxis in certain other situations, as determined by healthcare providers.
## Adult Dosing
* **Post-exposure prophylaxis:** 1 mL intramuscularly (IM) as a single dose.
* **Neonates born to HBsAg-positive mothers:** 0.5 mL IM within 12 hours of birth. This is typically given concurrently with the first dose of hepatitis B vaccine.
## Pediatric Dosing
* **Neonates born to HBsAg-positive mothers:** 0.5 mL IM within 12 hours of birth.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment. Dosing is based on exposure type and patient age (neonate).
## Contraindications
* Hypersensitivity to human immunoglobulin products or any component of the formulation.
* Individuals with a history of severe allergic reaction to human immunoglobulin.
## Adverse Effects
Common adverse effects include pain, redness, swelling, and itching at the injection site. Systemic reactions such as fever, headache, dizziness, nausea, and rash can also occur. Allergic reactions, including anaphylaxis, are rare but serious.
## Key Drug Interactions
* Hepatitis B vaccine: Can be administered concurrently, but at different injection sites. Concurrent administration may slightly reduce the immune response to the vaccine.
* Live virus vaccines: Avoid administering live virus vaccines for at least 3 months after HBIG administration, as HBIG may interfere with the immune response to these vaccines.
## Monitoring
* Monitor for signs of allergic reaction or anaphylaxis following administration.
* Monitor HBV serological markers (HBsAg, anti-HBs) as clinically indicated to confirm prophylaxis success.
## Clinical Pearls
* Administer IM. Do not administer intravenously.
* For neonates, concurrent administration with the first dose of hepatitis B vaccine is recommended.
* The protective effect of HBIG is transient. Vaccination is crucial for long-term immunity.
