# Tabovoihep
## Overview
Tabovoihep is a brand name for a combination product containing tenofovir disoproxil fumarate (TDF) and emtricitabine. It is a nucleoside reverse transcriptase inhibitor (NRTI) combination used for the treatment of HIV-1 infection.
## Primary Indications
Treatment of HIV-1 infection in adults and pediatric patients.
## Adult Dosing
The recommended dose is one tablet (containing TDF 300 mg and emtricitabine 200 mg) orally once daily.
## Pediatric Dosing
For pediatric patients weighing at least 17 kg to less than 22 kg: 150 mg TDF / 100 mg emtricitabine orally once daily.
For pediatric patients weighing at least 22 kg to less than 28 kg: 200 mg TDF / 133 mg emtricitabine orally once daily.
For pediatric patients weighing at least 28 kg to less than 35 kg: 250 mg TDF / 167 mg emtricitabine orally once daily.
For pediatric patients weighing 35 kg or more: The adult dose of one tablet (300 mg TDF / 200 mg emtricitabine) orally once daily.
*Note: Fixed-dose combination tablets for pediatric dosing are not available for all weight bands. Specific pediatric formulations or crushable tablets may be required.*
## Dose Adjustments
No dose adjustment is required for patients with mild to moderate renal impairment.
In patients with severe renal impairment (CrCl < 30 mL/min), the dose should be adjusted. Consult product labeling for specific adjustments.
In patients with hepatic impairment, no dose adjustment is generally recommended.
## Contraindications
Hypersensitivity to emtricitabine, tenofovir disoproxil fumarate, or any of its components.
## Adverse Effects
Common adverse effects include nausea, diarrhea, headache, rash, and dizziness.
More serious adverse effects can include lactic acidosis, severe hepatomegaly with steatosis, renal impairment (including acute renal failure and Fanconi syndrome), and bone mineral density loss.
## Key Drug Interactions
* **Nephrotoxic agents:** Concurrent use with other nephrotoxic drugs (e.g., cidofovir, NSAIDs) may increase the risk of renal toxicity.
* **Didanosine:** Coadministration with didanosine may increase didanosine plasma concentrations and reduce CD4 cell counts.
* **Atazanavir/Ritonavir:** Coadministration with atazanavir/ritonavir may increase tenofovir plasma concentrations.
## Monitoring
* Regular monitoring of renal function (serum creatinine, urine protein, urine glucose) is crucial, especially in patients with pre-existing renal disease or those taking other nephrotoxic agents.
* Monitor liver function tests.
* Monitor for signs and symptoms of lactic acidosis.
* Bone mineral density monitoring may be considered.
## Clinical Pearls
Tabovoihep is a once-daily, fixed-dose combination. It should be taken orally with or without food. Due to the risk of renal toxicity, careful monitoring of renal function is essential. Discontinuation may be necessary if significant renal impairment develops.
