# Tabovoihep
## Overview
Tabovoihep is a brand name for an oral formulation of tenofovir disoproxil fumarate (TDF), a nucleotide analog reverse transcriptase inhibitor (NtRTI).
## Primary Indications
Tabovoihep is indicated for the treatment of chronic hepatitis B virus (HBV) infection in adults and pediatric patients.
## Adult Dosing
The recommended dose of Tabovoihep for adults with chronic hepatitis B is **300 mg once daily**.
## Pediatric Dosing
The recommended dose of Tabovoihep for pediatric patients (12 years of age and older and with at least 35 kg body weight) with chronic hepatitis B is **300 mg once daily**.
## Dose Adjustments
No dose adjustment is typically required for patients with renal impairment, however, caution and potential dose adjustment may be necessary if creatinine clearance is significantly reduced. Refer to specific renal impairment guidelines. No dose adjustment is needed for hepatic impairment.
## Contraindications
Hypersensitivity to tenofovir disoproxil fumarate or any component of the formulation.
## Adverse Effects
Common adverse effects include nausea, diarrhea, vomiting, abdominal pain, headache, dizziness, rash, and fatigue.
Serious adverse effects can include lactic acidosis, severe hepatomegaly with steatosis, bone mineral density reduction (osteomalacia, osteoporosis), and renal impairment (including acute renal failure and Fanconi syndrome).
## Key Drug Interactions
* **Didanosine:** Concomitant use with tenofovir disoproxil fumarate may increase the risk of didanosine-related adverse events, including pancreatitis and peripheral neuropathy. Dose adjustment of didanosine is recommended.
* **Nephrotoxic agents:** Concomitant or sequential use with other nephrotoxic drugs (e.g., aminoglycosides, NSAIDs, foscarnet, ciclosporin) may increase the risk of renal-related adverse events.
## Monitoring
* **Renal function:** Baseline and periodic monitoring of serum creatinine, creatinine clearance, and urine analysis is crucial.
* **Liver function:** Baseline and periodic monitoring of ALT, AST, bilirubin, and HBV DNA levels.
* **Bone mineral density:** Consider monitoring in patients with a history of fractures or other risk factors for osteoporosis.
* **Lactate levels:** Monitor for signs and symptoms of lactic acidosis, especially in patients with risk factors.
## Clinical Pearls
* Tabovoihep is a component of various combination antiretroviral therapies for HIV, but this specific formulation (Tabovoihep) is primarily indicated for HBV. Ensure correct indication is being treated.
* Hepatitis B virus (HBV) should be tested for coinfection with HIV before initiating therapy.
* Drug-induced liver injury, including severe hepatomegaly with steatosis, has been reported.
* Ensure adequate hydration to minimize the risk of renal adverse events.
* Discontinuation of therapy may be associated with HBV exacerbations.
