## Overview
Tebipenem pivoxil is a carbapenem antibiotic administered orally. It is a prodrug that is hydrolyzed to its active form, tebipenem.
## Primary Indications
* Acute bacterial exacerbations of chronic bronchitis (AECB)
* Complicated urinary tract infections (cUTI), including pyelonephritis
## Adult Dosing
* **AECB:** 600 mg orally every 8 hours for 5 days.
* **cUTI:** 600 mg orally every 8 hours for 10 days.
## Pediatric Dosing
The safety and efficacy in pediatric patients have not been established.
## Dose Adjustments
No dose adjustment is typically required for renal impairment, though caution is advised and monitoring may be warranted in severe impairment.
## Contraindications
* Hypersensitivity to the active component, other carbapenem antibiotics, or any component of the formulation.
* Known severe hypersensitivity to beta-lactam antibiotics.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, headache, rash.
* **Serious:** Hypersensitivity reactions, *Clostridioides difficile*-associated diarrhea.
## Key Drug Interactions
* **Valproic acid:** Tebipenem pivoxil may decrease valproic acid concentrations, potentially leading to loss of seizure control. Coadministration is not recommended.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for diarrhea, especially if severe or persistent, and consider *C. difficile* testing.
## Clinical Pearls
* Tebipenem pivoxil is the first oral carbapenem antibiotic.
* It should be taken with food to improve absorption.
* Due to the risk of reduced efficacy, concurrent use with valproic acid is not recommended.
