# TAFOLAN Voihep
## Overview
Tafolan Voihep is a brand name for sofosbuvir, an antiviral medication used to treat chronic hepatitis C virus (HCV) infection. It is a nucleotide analog inhibitor of the HCV NS5B polymerase.
## Primary Indications
Chronic Hepatitis C virus (HCV) infection in combination with other antiviral agents. Specific genotypes and treatment durations depend on the regimen and prior treatment history.
## Adult Dosing
The standard dose is **400 mg orally once daily**.
The duration of treatment typically ranges from 8 to 24 weeks, depending on the HCV genotype, prior treatment experience, and presence of cirrhosis.
## Pediatric Dosing
Sofosbuvir is approved for use in pediatric patients 3 years of age and older. Dosing is weight-based:
* **17 kg to less than 30 kg:** 200 mg orally once daily
* **30 kg or greater:** 400 mg orally once daily
The duration of treatment is typically 12 weeks.
## Dose Adjustments
No dose adjustment is required for patients with mild, moderate, or severe renal impairment or hepatic impairment. However, caution is advised in severe renal impairment.
## Contraindications
* Hypersensitivity to sofosbuvir or any excipient in the formulation.
* Use in combination with P-glycoprotein (P-gp) inducers such as rifampin and St. John's Wort, as this can significantly reduce plasma concentrations of sofosbuvir, leading to loss of efficacy.
