# Tab volihep
## Overview
Tab volihep (volixibat) is a potent, non-bile acid, farnesoid X receptor (FXR) antagonist. It is thought to reduce bile acid synthesis and transport.
## Primary Indications
Currently, tab volihep is **not approved** by the FDA for any indication. It has been investigated in clinical trials for conditions such as primary biliary cholangitis (PBC) and nonalcoholic steatohepatitis (NASH).
## Adult Dosing
Dosing regimens in clinical trials varied. For example, in a Phase 2 trial for PBC, doses of 5 mg, 10 mg, and 20 mg once daily were evaluated. The optimal dose and frequency for any potential indication are not established.
## Pediatric Dosing
There is **no established pediatric dosing** for tab volihep.
## Dose Adjustments
Dose adjustments are **not established** as there is no approved indication. In clinical trials, dose modifications or discontinuations were based on the emergence of adverse events, particularly pruritus.
## Contraindications
Contraindications are **not established** due to the lack of approved indications. However, caution would likely be advised in patients with severe hepatic impairment.
## Adverse Effects
The most commonly reported adverse effect in clinical trials was **pruritus** (itching), which could be dose-dependent and severe. Other potential adverse effects may include diarrhea, abdominal pain, and nausea.
## Key Drug Interactions
Specific drug interactions are **not well-established** for tab volihep. As an FXR antagonist, it may theoretically impact the metabolism of other drugs, but clinical significance is unknown.
## Monitoring
Monitoring parameters are **not established**. In clinical trials, liver function tests and assessment for pruritus were typically performed.
## Clinical Pearls
* Tab volihep is an investigational agent and is not currently available for clinical use.
* The most significant adverse event observed in trials was pruritus.
* Efficacy and safety for any specific indication are not definitively proven.
