# Tabvoihep
## Overview
Tabvoihep is a brand name for Hepatitis B Immune Globulin (HBIG). It is a sterile solution of immunoglobulin G (IgG) containing antibodies against the hepatitis B surface antigen (HBsAg).
## Primary Indications
* **Post-exposure prophylaxis against Hepatitis B infection:** Administered after exposure to the hepatitis B virus (HBV), such as needlestick injuries or sexual exposure to an HBsAg-positive individual.
* **Prevention of HBV recurrence in liver transplant recipients:** Used in conjunction with hepatitis B vaccine to prevent the re-emergence of HBV infection in the transplanted liver.
## Adult Dosing
* **Post-exposure prophylaxis:**
* **Peripherally exposed individuals (e.g., needlestick):** 0.06 mL/kg IM in a single dose.
* **Sexually assaulted individuals:** 0.06 mL/kg IM in a single dose. This should be given as soon as possible, preferably within 14 days of exposure. Vaccination should also be initiated concurrently.
* **Liver transplant recipients:** Dosing is highly individualized and depends on local transplant center protocols, often involving frequent dosing (e.g., daily or weekly) post-transplant, sometimes in combination with antiviral therapy.
## Pediatric Dosing
* **Post-exposure prophylaxis:**
* **Infants born to HBsAg-positive mothers:** 0.5 mL IM within 12 hours of birth. Hepatitis B vaccine series should also be initiated concurrently.
* **Other pediatric exposures:** 0.06 mL/kg IM in a single dose.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment, but caution is advised in patients with a history of severe allergic reactions to human immune globulin.
## Contraindications
* Known history of anaphylactic or severe systemic reaction to human immunoglobulin preparations.
* IgA deficiency with known antibody against IgA.
## Adverse Effects
Common adverse effects include injection site pain, redness, and swelling. Less common effects may include headache, fever, chills, muscle aches, fatigue, nausea, and dizziness. Severe reactions, including anaphylaxis, are rare but possible.
## Key Drug Interactions
* **Live virus vaccines:** Administration of HBIG may interfere with the immune response to live virus vaccines. Vaccination should be deferred for at least 3 months after HBIG administration.
* **Other immunoglobulins:** Avoid co-administration if possible.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* For transplant recipients, monitor HBV serological markers (HBsAg, anti-HBs, HBV DNA) as per transplant center protocol.
## Clinical Pearls
* HBIG provides passive immunity and should be administered as soon as possible after exposure.
* For post-exposure prophylaxis in adults, concurrent administration of the hepatitis B vaccine series is recommended.
* The 0.06 mL/kg dose for adults can be administered intramuscularly in a single injection.
