# Tabvoihep
## Overview
Tabvoihep is a brand name for the combination of **lamivudine and tenofovir disoproxil fumarate (TDF)**. It is a nucleoside/nucleotide reverse transcriptase inhibitor (NRTI) combination antiretroviral medication used for the treatment of HIV-1 infection.
## Primary Indications
Treatment of HIV-1 infection in adults and pediatric patients (2 years of age and older) with evidence of viral suppression or as part of a complete regimen for initial therapy.
## Adult Dosing
* **One tablet (lamivudine 300 mg / tenofovir disoproxil fumarate 300 mg) orally once daily.**
## Pediatric Dosing
* **Children weighing at least 25 kg to less than 33 kg:** One half tablet (lamivudine 150 mg / tenofovir disoproxil fumarate 150 mg) orally once daily.
* **Children weighing 33 kg or more:** One tablet (lamivudine 300 mg / tenofovir disoproxil fumarate 300 mg) orally once daily.
*Note: Pediatric dosing is based on weight. If a specific weight-based dosing is not available for Tabvoihep, consider using individual components of lamivudine and tenofovir disoproxil fumarate according to weight-based guidelines.*
## Dose Adjustments
* **Renal Impairment (Adults):** Dose adjustment of lamivudine and tenofovir disoproxil fumarate is required. Consult specific guidelines for lamivudine and tenofovir disoproxil fumarate dose adjustments based on creatinine clearance. Tabvoihep is **not recommended** for patients with a creatinine clearance < 30 mL/min.
* **Hepatic Impairment:** No dose adjustment is generally required.
## Contraindications
* Hypersensitivity to lamivudine, tenofovir disoproxil fumarate, or any component of the tablet.
## Adverse Effects
Common adverse effects include nausea, diarrhea, headache, dizziness, rash, and fatigue. More serious adverse effects include lactic acidosis, severe hepatomegaly with steatosis, and renal impairment (especially with tenofovir disoproxil fumarate).
## Key Drug Interactions
* **Didanosine:** Concomitant use with tenofovir disoproxil fumarate may increase the risk of didanosine-related toxicities.
* **Antiretrovirals:** Avoid concomitant use with adefovir dipivoxil.
* **Nephrotoxic Agents:** Caution with concurrent use of other nephrotoxic drugs (e.g., NSAIDs, certain antibiotics) due to increased risk of renal adverse events.
* **Herbal Products:** St. John's Wort may decrease plasma concentrations of lamivudine and tenofovir, reducing efficacy.
## Monitoring
* **Renal Function:** Monitor serum creatinine and estimated creatinine clearance regularly, especially in patients with pre-existing renal disease or those at risk for renal toxicity.
* **Liver Function:** Monitor liver function tests (AST, ALT, bilirubin) regularly, especially in patients with pre-existing liver disease or risk factors for liver disease.
* **Viral Load and CD4 Count:** Monitor HIV-1 RNA levels and CD4+ T-cell counts to assess treatment efficacy.
* **Bone Mineral Density:** Consider monitoring bone mineral density in patients with a history of fractures or other risk factors for osteoporosis.
## Clinical Pearls
* Tabvoihep is a fixed-dose combination and should not be used in patients who require dose adjustments of either component.
* Ensure patients are counseled on the importance of adherence to antiretroviral therapy to prevent resistance.
* Tenofovir disoproxil fumarate has been associated with renal and bone toxicity. Patients should be assessed for risk factors and monitored accordingly.
