## Primary Indications
* Treatment of chronic hepatitis B virus (HBV) infection in adults and pediatric patients.
## Adult Dosing
* **Recommended Dose:** 300 mg orally once daily.
## Pediatric Dosing
* **Ages 12 to <18 years:** 300 mg orally once daily.
* **Ages 6 to <12 years:** 10 mg/kg orally once daily, not to exceed 300 mg.
* **Ages <6 years:** Efficacy and safety have not been established.
## Dose Adjustments
* **Renal Impairment:** Dose adjustments are necessary based on estimated creatinine clearance (CrCl). Consult prescribing information for specific recommendations.
* CrCl 30 to <50 mL/min: 300 mg every 48 hours.
* CrCl <30 mL/min or on hemodialysis: Consult prescribing information for individualized dosing and frequency.
## Contraindications
* Hypersensitivity to tenofovir disoproxil fumarate or any component of the formulation.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, headache, abdominal pain, rash, asthenia.
* **Serious:** Lactic acidosis, severe hepatomegaly with steatosis, renal impairment (including acute renal failure and proximal tubulopathy), bone mineral density decrease, new-onset or worsening of renal impairment.
## Key Drug Interactions
* **Nephrotoxic Agents:** Concomitant use with drugs that are nephrotoxic or may reduce renal function (e.g., aminoglycosides, NSAIDs, certain antiretrovirals) may increase the risk of renal adverse events. Separate administration or monitor renal function closely.
* **Lamiuvdine, Emtricitabine, Adefovir, Cidofovir:** Avoid coadministration due to increased risk of renal toxicity.
## Monitoring
* **Renal Function:** Baseline and periodic monitoring of serum creatinine, BUN, and urinalysis, particularly in patients with pre-existing renal disease or those receiving concomitant nephrotoxic agents.
* **Hepatic Function:** Baseline and periodic monitoring of liver enzymes (ALT, AST), bilirubin, and HBV DNA levels.
* **Bone Mineral Density:** Consider monitoring in patients with a history of pathological fractures or other risk factors for osteoporosis.
## Clinical Pearls
* Tenofovir disoproxil fumarate (TDF) should be taken orally once daily with or without food.
* Patients should be advised to report any signs of new or worsening kidney problems (e.g., decreased urination, fatigue, weakness) or bone pain.
* Discontinuation may lead to HBV flare. Monitor liver function closely after discontinuation.
