# Tabovoihep
## Overview
Tabovoihep is a brand name for tenofovir alafenamide (TAF) in combination with emtricitabine (FTC). It is a nucleotideresponsive reverse transcriptase inhibitor (NRTI) combination used for the treatment of HIV-1 infection. TAF has a more targeted delivery to lymphoid cells and lower plasma tenofovir concentrations compared to tenofovir disoproxil fumarate (TDF), potentially leading to improved renal and bone safety profiles.
## Primary Indications
Treatment of human immunodeficiency virus type 1 (HIV-1) infection in adults and pediatric patients at least 12 years of age and weighing at least 35 kg, as part of combination antiretroviral treatment.
## Adult Dosing
* **Recommended Dose:** One tablet (200 mg emtricitabine / 25 mg tenofovir alafenamide) orally once daily.
## Pediatric Dosing
* **Recommended Dose:** One tablet (200 mg emtricitabine / 25 mg tenofovir alafenamide) orally once daily for pediatric patients at least 12 years of age and weighing at least 35 kg.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is recommended for mild to moderate renal impairment (eGFR ≥ 30 mL/min). Safety and efficacy in severe renal impairment (eGFR < 30 mL/min) have not been established.
* **Hepatic Impairment:** No dose adjustment is recommended for mild to moderate hepatic impairment. Data in severe hepatic impairment are limited.
## Contraindications
* Hypersensitivity to emtricitabine, tenofovir alafenamide, or any component of the formulation.
## Adverse Effects
* **Common:** Diarrhea, nausea, headache, fatigue, dizziness, rash, abdominal pain.
* **Less Common/Serious:** Lactic acidosis, severe hepatomegaly with steatosis, immune reconstitution inflammatory syndrome (IRIS), renal impairment, bone density decrease.
## Key Drug Interactions
* **Strong CYP3A4 Inducers:** (e.g., rifampin, carbamazepine, phenytoin, St. John's Wort) can decrease plasma concentrations of TAF/FTC, potentially leading to loss of virologic response. Concomitant use is not recommended.
* **Strong CYP3A4 Inhibitors:** (e.g., ketoconazole, ritonavir, cobicistat) may increase TAF/FTC concentrations. Monitor for adverse effects.
* **Antacids:** Separate administration by at least 2 hours.
* **Other Antiretrovirals:** Consult specific combination guidelines.
## Monitoring
* **Baseline:** HIV-1 RNA, CD4 count, serum creatinine and eGFR, electrolytes, liver function tests (AST, ALT), CBC. Consider bone mineral density.
* **Regularly:** HIV-1 RNA, CD4 count, serum creatinine and eGFR, electrolytes, liver function tests.
## Clinical Pearls
* Tabovoihep should be taken with or without food.
* This fixed-dose combination contains tenofovir alafenamide, which has a different pharmacokinetic profile than tenofovir disoproxil fumarate.
* Patients should be tested for Hepatitis B virus (HBV) infection before initiating treatment, as discontinuation of Tabovoihep may result in severe exacerbation of HBV.
* Educate patients on potential renal and bone adverse effects, and the importance of adherence.
