# Tavabolep (Tavabolep)
## Overview
Tavabolep is a medication used in the management of hypercholesterolemia. It works by inhibiting cholesterol absorption in the small intestine.
## Primary Indications
* **Hypercholesterolemia:** As an adjunct to diet to reduce elevated total cholesterol, LDL-cholesterol, apolipoprotein B, and non-HDL-cholesterol in patients with primary hypercholesterolemia.
* **Homozygous Familial Hypercholesterolemia (HoFH):** To reduce elevated total cholesterol and LDL-cholesterol in patients with HoFH.
## Adult Dosing
* **Primary Hypercholesterolemia:** The recommended dose is 10 mg once daily.
* **Homozygous Familial Hypercholesterolemia (HoFH):** The recommended dose is 10 mg once daily.
## Pediatric Dosing
* **Primary Hypercholesterolemia:** Safety and effectiveness have been established in pediatric patients aged 10 to 17 years. Dosing is generally 10 mg once daily.
* **Homozygous Familial Hypercholesterolemia (HoFH):** Safety and effectiveness have not been established in pediatric patients for HoFH.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is generally required for mild to moderate renal impairment. Limited data in severe renal impairment; use with caution.
* **Hepatic Impairment:** No dose adjustment is generally required for mild hepatic impairment. Use with caution in moderate hepatic impairment and avoid in severe hepatic impairment.
## Contraindications
* Hypersensitivity to tavabolep or any of its excipients.
* Active liver disease or unexplained persistent elevations of serum transaminases.
## Adverse Effects
The most common adverse effects include:
* Diarrhea
* Abdominal pain
* Fatigue
* Arthralgia
* Cough
* Upper respiratory tract infection
## Key Drug Interactions
* **Cholestyramine, Colestipol, Colesevelam:** Coadministration may decrease tavabolep bioavailability. Administer tavabolep at least 2 hours before or 4 hours after these bile acid sequestrants.
* **Fibrates:** Increased risk of cholelithiasis and cholecystitis.
* **Cyclosporine:** May increase tavabolep concentrations. Monitor for potential toxicity.
## Monitoring
* **Liver Function Tests:** Baseline liver function tests (ALT, AST) are recommended before initiating therapy. Monitor periodically, especially if elevations are noted.
* **Lipid Profile:** Monitor lipid levels as clinically indicated.
## Clinical Pearls
* Tavabolep should be taken once daily, with or without food.
* It should be used in conjunction with a cholesterol-lowering diet.
* The drug's effectiveness in reducing cardiovascular events has not been established.
