# **Tebipenem Pivoxil Hydrobromide**
## Overview
Tebipenem pivoxil hydrobromide is an oral carbapenem antibiotic. It is a prodrug that is rapidly hydrolyzed to its active form, tebipenem.
## Primary Indications
Tebipenem pivoxil hydrobromide is indicated for the treatment of acute bacterial exacerbations of chronic bronchitis (AECB) in adults.
## Adult Dosing
The recommended dose is 600 mg orally every 8 hours for 5 days.
## Pediatric Dosing
Tebipenem pivoxil hydrobromide is not approved for use in pediatric patients.
## Dose Adjustments
No dose adjustment is required for patients with renal impairment, as tebipenem is not significantly cleared by the kidneys.
## Contraindications
Tebipenem pivoxil hydrobromide is contraindicated in patients with:
* Known severe hypersensitivity to the active component, other carbapenem antibacterial drugs, or any component of the formulation.
## Adverse Effects
Common adverse effects include:
* Diarrhea
* Nausea
* Vomiting
* Abdominal pain
* Rash
* Headache
Serious adverse effects include:
* Hypersensitivity reactions, including anaphylaxis
* Clostridioides difficile-associated diarrhea
* Seizures (rare)
## Key Drug Interactions
* **Valproic acid:** Tebipenem pivoxil hydrobromide can decrease serum valproic acid concentrations, potentially leading to loss of seizure control. Monitor valproic acid levels closely and consider alternative antibiotic therapy.
* **Probenecid:** Probenecid can increase tebipenem plasma concentrations. Coadministration is not recommended.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for signs and symptoms of C. difficile infection.
* If co-administered with valproic acid, monitor valproic acid serum concentrations.

* Tebipenem pivoxil hydrobromide is the first oral carbapenem antibiotic available in some markets.
* It has broad-spectrum activity against many Gram-positive and Gram-negative bacteria, including some resistant organisms.
* Due to its potential to decrease valproic acid levels, it should be used with caution or avoided in patients taking valproic acid for seizure disorders.
