# Tabravihiep (Tavaborole)
## Overview
Tavaborole is an oxaborole antifungal agent that inhibits fungal protein synthesis by inhibiting leucyl-tRNA synthetase. It is formulated as a 5% topical solution for the treatment of onychomycosis.
## Primary Indications
Onychomycosis of the toenails due to *Trichophyton rubrum* or *Trichophyton mentagrophytes*.
## Adult Dosing
Apply to the affected toenail(s) once daily. Use the provided applicator to cover the entire nail plate and under the free edge of the nail.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 6 years. For patients ≥6 years, dosing is the same as adults (apply once daily).
## Dose Adjustments
No specific dosage adjustments are required for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
Hypersensitivity to tavaborole or any component of the formulation.
## Adverse Effects
- Application site exfoliation
- Ingrown toenail (onychocryptosis)
- Application site erythema
- Application site dermatitis
## Key Drug Interactions
None significant due to low systemic absorption and lack of systemic metabolic pathways when applied topically.
## Monitoring
- Monitor for signs of local irritation or worsening of the infection.
- Clinical efficacy should be evaluated after 48 weeks of treatment.
## Clinical Pearls
- Treatment requires up to 48 weeks of once-daily application.
- The nail must be clean and dry prior to application.
- Avoid contact with eyes, mouth, or vagina.
- If no clinical improvement is observed after 48 weeks, consider alternative treatments.
- Efficacy is generally lower in distal subungual onychomycosis with significant involvement of the nail matrix (lunula).
