# Sompraz hp
## Overview
Sompraz hp is a fixed-dose combination product containing vonoprazan, amoxicillin, and clarithromycin. It is a regimen for the eradication of *Helicobacter pylori*.
## Primary Indications
* Treatment of *Helicobacter pylori* infection in adults.
## Adult Dosing
The standard adult dose is **one kit taken twice daily for 14 days**. Each kit contains:
* Vonoprazan 20 mg
* Amoxicillin 500 mg
* Clarithromycin 250 mg
## Pediatric Dosing
Dosing for pediatric patients has not been established.
## Dose Adjustments
No specific dose adjustments are listed for renal or hepatic impairment, but caution is advised.
## Contraindications
* Known hypersensitivity to vonoprazan, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of cholestatic jaundice or hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with cisapride, pimozide, and other drugs with a narrow therapeutic index that are substrates of CYP3A4.
* Severe renal impairment (CrCl < 30 mL/min) for vonoprazan component.
## Adverse Effects
Common adverse effects include:
* Diarrhea
* Abdominal pain
* Nausea
* Vomiting
* Taste disturbance
* Headache
More serious adverse effects can include:
* Allergic reactions (including anaphylaxis)
* Clostridioides difficile-associated diarrhea
* QT prolongation (associated with clarithromycin)
* Hepatotoxicity
* *Pseudomembranous colitis*
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Clarithromycin is a moderate CYP3A4 inhibitor. Vonoprazan is also metabolized by CYP3A4. Concomitant use with strong CYP3A4 inhibitors may increase exposure to clarithromycin and vonoprazan.
* **Warfarin:** Increased risk of bleeding.
* **Colchicine:** Increased risk of colchicine toxicity.
* **Digoxin:** Increased digoxin levels.
* **H2-receptor antagonists and antacids:** May decrease absorption of vonoprazan.
* **Drugs that prolong the QT interval:** Increased risk of ventricular arrhythmias.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Assess for and manage gastrointestinal side effects.
* Monitor for effectiveness of *H. pylori* eradication (usually via stool antigen test or urea breath test after treatment completion).
## Clinical Pearls
* Take the medications as directed for the full 14-day course, even if symptoms improve.
* If a dose is missed, take it as soon as remembered, unless it is almost time for the next dose. Do not double doses.
* Inform patients about potential side effects and when to seek medical attention.
