# Sompraz hp
## Overview
Sompraz hp is a combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*.
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
**Sompraz hp** is typically taken as follows for 7 to 14 days:
* One packet in the morning and one packet in the evening.
Each packet contains pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 500 mg.
## Pediatric Dosing
Dosing in pediatric patients is not established for this combination product. Refer to individual agent guidelines.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment for pantoprazole is recommended in mild to moderate renal impairment. In severe renal impairment, caution and potential dose reduction may be considered, though specific guidelines are limited for the combination. Amoxicillin and clarithromycin doses may need adjustment based on renal function.
* **Hepatic Impairment:** Pantoprazole may require dose adjustment in severe hepatic impairment; typically a 20 mg daily dose is recommended. Caution and potential dose reduction for clarithromycin are advised in moderate to severe hepatic impairment. Amoxicillin does not typically require dose adjustment for hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of allergic reactions to penicillins, cephalosporins, or macrolides.
* Patients with a history of cholestatic jaundice/hepatic dysfunction associated with prior macrolide exposure.
* Concomitant use with cisapride, ergot alkaloids, pimozide, or simvastatin/lovastatin (due to clarithromycin).
* Severe renal impairment with concomitant use of certain medications metabolized by CYP3A4 where amoxicillin dose adjustment might be needed.
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, taste disturbance, headache, rash, and yeast infections. Serious adverse effects may include severe allergic reactions, *Clostridioides difficile*-associated diarrhea, liver dysfunction, QT prolongation, and tendon rupture.
## Key Drug Interactions
* **Clarithromycin:** Interacts with numerous CYP3A4 substrates (e.g., statins, anticoagulants, certain antiretrovirals, calcium channel blockers), increasing their plasma concentrations and risk of toxicity.
* **Pantoprazole:** May decrease the absorption of drugs that require acidic gastric pH (e.g., ketoconazole, itraconazole, digoxin, certain HIV medications). May interact with methotrexate.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid may decrease renal excretion of amoxicillin.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for diarrhea, especially *Clostridioides difficile*-associated diarrhea.
* Monitor liver function tests, particularly with prolonged use or in patients with pre-existing liver disease.
* Monitor for signs of QT prolongation with clarithromycin, especially in patients with risk factors.
## Clinical Pearls
* This regimen is typically taken for 7 to 14 days. The duration of therapy is critical for successful *H. pylori* eradication.
* Administer pantoprazole 30-60 minutes before meals.
* Patients should be advised to complete the full course of treatment even if symptoms improve.
* Ensure patients are not allergic to penicillin or macrolide antibiotics.
